Essential Duties and Responsibilities:
The Veteran Outreach Program Case Manager is a crucial role within the organization, responsible for providing comprehensive case management services to military veterans and their families. The primary objective of this position is to assist veterans in accessing and navigating various resources, services, and benefits available to them, ultimately promoting their overall well-being and successful reintegration into civilian life. The Case Manager will work closely with veterans, community agencies, and government entities to assess needs, develop individualized care plans, and ensure the effective delivery of support services.
- Maintain accurate client data in the agency database, ETO, ECM and county database HMIS daily.
- Promote the Veteran Outreach Program by participating in community events, workshops, and conferences, raising awareness of available services and engaging potential clients.
- Conduct thorough assessments of veterans' needs, including physical, emotional, social, and economic aspects. Identify any mental health challenges, physical disabilities, employment barriers, or other issues that require attention.
- Evaluating veterans and their families to determine their eligibility for SSVF assistance based on income and other program requirements.Identifying and engaging with homeless or at-risk veteran families to inform them about the available SSVF services and assess their needs.
- Collaborate with veterans to develop comprehensive, tailored care plans that address their unique needs and goals. These plans may involve access to medical care, housing assistance, job training, mental health counseling, substance abuse treatment, financial aid, and other relevant services.
- Regularly monitor veterans' progress in achieving their goals and ensure that they receive ongoing support. Conduct follow-up meetings and check-ins to assess the effectiveness of interventions and modify care plans as needed.
- Serve as a strong advocate for veterans, ensuring that their rights are protected and they receive the support they deserve. Interact with government agencies, employers, and community partners on behalf of veterans to secure necessary services and benefits.
- Connect veterans with appropriate resources and support networks, both within the organization and in the wider community. Maintain an up-to-date database of local resources, organizations, and government agencies that can provide assistance to veterans.
- Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, and Trauma Informed Care.
- Make warm handoff referrals to outside agencies that can benefit the client in reaching their individualized service plan goals.
- Transports clients in the agency-provided vehicle for appointments as required.
- Meeting with clients face-to-face in their homes, shelters, or other environments.
- Actively engages in collaborative efforts with fellow staff members to ensure that clients receive the necessary support they require.
- Maintain high ethical standards in all aspects of the role. Adhere to professional codes of conduct, respect client confidentiality, and prioritize the well-being and rights of clients at all times.
- Be prepared to respond to crisis situations promptly and effectively, providing immediate assistance and access to appropriate resources when needed.
- Responsible for promptly notifying the manager of any work orders that need to be submitted or supplies that need to be ordered.
- Responsible for attending staff meetings, ensuring timely completion of training, and accurately completing timecards.
- Perform related duties as assigned by the supervisor.
- Maintain compliance with all company policies and procedures.
Education and/or Work Experience Requirements:

- High School Diploma or equivalent: A minimum educational requirement to demonstrate basic literacy and communication skills.
- Associate degree required.
- Veteran status preferred.
- Experience in case management, particularly with veteran populations, is preferred.
- Knowledge of military culture, veteran-specific issues, and the challenges veterans may face during their transition to civilian life.
- Strong interpersonal and communication skills to establish rapport with veterans, families, and community partners.
- Crisis intervention skills: Capacity to handle crisis situations and de-escalate conflicts.
- Empathy and compassion: Demonstrating genuine care and understanding for individuals experiencing homelessness.
- Ability to work in a team: Collaboration and cooperation with other staff, volunteers, and external agencies.
- Excellent computer proficiency (MS Office – Word, Excel and Google)
Required Qualifications:
Employment Eligibility Verification:
- Must provide documentation to establish identity and employment eligibility in compliance with legal requirements. This may include a U.S. passport, Permanent Resident Card, or other acceptable forms of identification.
Negative Tuberculosis Test:
- Must provide documentation of a recent negative tuberculosis (TB) test result to ensure a safe and healthy work environment.
Background Screening:
- Successful completion of a comprehensive background screening, which may include criminal record checks, employment history verification, and reference checks.
Pre-Employment Drug Screening:
- Must pass a pre-employment drug screening to ensure a drug-free workplace. A negative drug test result is required.
Driving Skills:
- Must meet the requirements set by our auto insurance carrier.
Insurance Coverage:
- Must be able to qualify for Good Samaritan Shelter insurance coverage, which may include meeting specific criteria related to driving record, insurance claims history, and other factors.
Personal Insurance Coverage:
- Must have personal insurance coverage that meets the requirements set by the employer.

The Supportive Services for Veteran Families (SSVF) program is a critical initiative aimed at providing housing stability and supportive services to very low-income veterans and their families who are experiencing homelessness or at risk of becoming homeless. SSVF program staff play a central role in this endeavor, working in a dynamic and client-centered environment. These dedicated professionals spend time both in an office setting, where they coordinate services, collaborate with community partners, and perform administrative tasks, as well as in the field, where they engage directly with veterans and their families. Through one-on-one interactions, SSVF program staff assess individual needs, develop personalized stability plans, and provide essential support to help veterans secure stable housing and access vital resources. The work environment can be fast-paced and challenging, but it is equally rewarding, as staff members make a significant impact on the lives of those who have served their country.
