The Manager of Case Management has operational accountability and oversight, and manages the daily activities of the department or responsible area and supports best practice standards for department functions and the Case Management Program. The Manager adheres to all Federal, State and regulatory statues and accreditation requirements. The Manager supports the Director in developing, planning and implementing appropriate changes in keeping with case management best practice standards and organizational goals.

Qualifications

We are seeking an Experienced Leader for a Manager of Case Management with the following qualifications:

· Graduate of an accredited School of Nursing or Social Work

· Registered Nurse (RN):BSN required; MSN preferred

· Social Work:MSW required

· Active State of Florida licensure required:

o RN license (nursing pathway), or

o LCSW or LMHC (social work pathway)

· Required certification:

o RN: Certified Case Manager (CCM) or Certified Advanced Case Manager (C-ACM)

o Social Work: Certified Social Work Case Manager (C-SWCM) or Certified Advanced Social Work Case Manager (C-ASWCM)

· 35 years of case management or leadership experience within healthcare

· Minimum 1 year of supervisory or managerial experience, preferably in an acute care hospital setting

Demonstrated expertise in hospital operations, discharge planning, care coordination and interdisciplinary collaboration.

Case Management Manager responsible for leading hospital-based case management operations, care coordination, utilization management, discharge planning, and interdisciplinary teams to ensure quality patient outcomes, regulatory compliance, and efficient resource utilization across the continuum of care.
