Full Job Description

We have an opening for Chronic Care Manager, LPN, in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. We do not work weekends or holidays. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork.

Our focus is in assisted living communities. We are offering a full time position to Chronic Care Managers, LPNs, to better serve our clients medical needs. Starting pay is $60k, but with our incentive pay structure, our CCMs average around $78k/year.

A chronic care manager does a combination of administrative work as well as case management. Job duties include but are not limited to; order processing, phone call communications with family and other providers, insurance processing, med reconciliation, case management. You are the direct liaison between our company and the facilities we service. You will be working directly with NPs that we hire through the company that provide primary care services to our patients.

Being a mobile practice we go out to Assisted Living communities to provide services. We go to 1 community each day. We have 2 buildings that will need coverage. One is O'Fallon and the other in Swansea, IL. We travel to 1 community per day, but all of our communities weekly. We are looking for someone willing to travel to both locations.

Due to the unique nature of this job, we do require our potential hires to shadow one of our current providers for a few hours in order to make sure it is a good fit for both parties. With this being a new area, the closest shadowing opportunity would be in either Centralia or Mount Vernon. Please ensure you are willing to participate in the shadow prior to applying.
