Full Job Description

Shift: 8-hour/day Mondays to Fridays (weekends coverage may be needed periodically)

SUMMARY OF POSITION:

Manage Client Caseload
Receives, reviews, and enters data into Database Management Software
Facilitate House Meetings and Social Enrichment Activities
Facilitate support group for adult residents
Complete intakes using Database Management Software
Participate in agency meetings
Deescalate and provide crisis intervention for emergencies in shelter
Handle day to day requests and situations

PRIMARY RESPONSIBILITIES:

Manage Client Caseload

Complete a Safety Plan for each client on caseload
Review new intakes to ensure all forms and Database Management Software data entry are completed and accurate.
Complete a BWCS Resident Assessment on all new clients on caseload.
Work collaboratively with clients on caseload to complete an Individual Service Plan.
Ensure client has completed a Wellness Check within 48 hours of intake (72 hours for clients arriving on the weekends/holidays).
Work collaboratively with clients on caseload to develop an action plan, goals, and complete objectives.
Act as a liaison between clients and other social services/community agencies.
Advocate for clients.
Actively listen to clients, providing information, support, and understanding.
Connect clients with community resources.
Provide information and referrals to community resources.
Maintain accurate client case files at all times.

Receives, reviews, and enters date into Database Management Software

Keeps track of services provided during case management sessions.
Enters service provision data using Data Management Software following format displayed on screen.
Confirms accuracy of data.
Reviews and makes necessary corrections to information entered.
Ensures all service provision is accurately entered daily.

Facilitate House Meetings and Social Enrichment Activities

Promote cooperative living through regular house meeting with residents.
Facilitate discussions between residents in order to promote cooperative living.
Provide information to residents pertaining to Daily Living Agreement, events at the shelter, etc.
Facilitate social enrichment activities with residents of shelter.

Facilitate support group for adult residents

Prepare topics for discussion for support group
Promote support groups times and locations through flyers, direct communication, etc.
Organize scheduling and location for support groups
Obtain support group client evaluations at the end of each session
Enter to Database Management Software all participants of group facilitated

SECONDARY RESPONSIBILITIES:

Complete intakes using Database Management Software

Completes timely and courteous intakes on new clients arriving during that shift when requested by Shelter Monitor (intake worker).
Assigns the case manager and file number to new clients immediately upon intake completion.
Updates Resident Enter/Exit log when completing an intake.
Ensures FVPS policies and procedures are followed at all time pertaining to case files, case manager assignment, case file distribution, and confidentiality.
Enters to Database Management Software any child entering the BWCS after adult resident’s intake date.

Participate in agency meetings

Communicate with agency staff during intra-agency meetings.
Attend staff meetings to review goals and objectives, action plan, and goal attainment of individual client cases.

Deescalate and provide crisis intervention for emergencies in shelter

Deescalate situations.
Crisis intervention.
Call 911 for fire, health and security emergencies and communicate with authorities about nature of the problem.
Administer CPR in appropriate circumstances.

Handle day to day requests and situations

Provide assistance in shelter monitor’s office, when requested.
Answer crisis calls.
Respond to crisis calls.
Assess clients’ needs.
Complete Crisis Log Intake form for each call.
Determine eligibility of caller for FVPS services.
Provide information and referrals for community resources.
Provide crisis intervention.
Other duties as assigned.

EDUCATION/EXPERIENCE REQUIREMENTS:

Education:

B.S.W. or B.A. degree in human services or related field
Related work experience may be substituted for a college degree

Experience:

Ability to work as a team member, providing support and constructive feedback in
Interpersonal interactions with both staff and residents.
Competency with computers and standard office equipment
Strong writing skills and knowledge of proper documentation techniques.
Knowledge and understanding of the effects of family violence at macro, mezzo, and micro levels.
Sensitivity to the needs of battered women and children.
Cultural Competency
Bilingual in English & Spanish (Preferred)
