About Acti-Kare Home Care - Puget Sound
Acti-Kare is a locally owned home care agency that has been serving families across the greater Seattle area since 2016. We provide in-home care for seniors and adults who need support with personal care, companionship, dementia care, mobility, daily activities, and remaining safely and independently at home.
We serve clients throughout King, Snohomish, and Pierce Counties, this role will serve all of these areas.

We are looking for a strong person to join our Client Care team.
About the Role
This is not a traditional desk-based coordinator position.
Our Client Care Coordinators are part relationship-builder, part care coordinator, part problem-solver, and part field leader.
You will often be one of the first people a family meets when they realize their parent, spouse, or loved one needs help at home. You need to be able to walk into that situation with confidence and empathy, ask thoughtful questions, understand what the family really needs, explain our services clearly, and help them feel comfortable taking the next step.
Client intake is part care assessment and part consultative sales. We are not looking for a high-pressure salesperson. We are looking for someone who builds trust naturally, listens carefully, communicates value, answers questions confidently, and is comfortable asking a family to move forward with care when Acti-Kare is the right solution.
Once care begins, you become an advocate for that client and an important connection between the family, caregivers, and our office team. You will help make sure the care we promised is the care we actually deliver.
This role also includes meaningful field support. Because of that, an active Washington State CNA credential and hands-on caregiving experience are required.
You should understand personal care, dementia care, mobility assistance, transfers, ADLs, caregiver scope of practice, and the realities caregivers encounter in a client's home. You will sometimes help caregivers understand a client's care needs, reinforce skills, and provide hands-on coaching within appropriate scope and agency policy.
WHAT YOU'LL DO
Client Intake & New Care
- Respond professionally and promptly to prospective clients and families
- Speak with families by phone to understand their situation, care needs, urgency, priorities, and concerns
- Schedule and conduct in-home consultations, client intakes, and assessments
- Build trust with families who may be overwhelmed, uncertain, or facing a sudden change in a loved one's health
- Explain our services, recommendations, pricing, and next steps clearly and confidently
- Help prospective clients move from inquiry to starting care when Acti-Kare is the right fit
- Develop individualized care plans based on the client's needs, routines, preferences, risks, and family concerns
- Coordinate with the office team to prepare for successful new client starts
Ongoing Client Care
- Manage an assigned caseload of home care clients
- Build strong ongoing relationships with clients and their families
- Conduct regular quality assurance visits and client check-ins
- Monitor client satisfaction, care quality, caregiver fit, and consistency of service
- Identify changes in condition, care needs, family expectations, or caregiver performance before they become larger problems
- Coordinate care plan and service changes with families, caregivers, scheduling, and other members of the office team
- Handle concerns professionally, calmly, and with a solutions-oriented mindset
- Maintain accurate, thorough, and compliant client records in our care management software
- Ensure adherence to Washington State home care regulations, caregiver scope of practice, and Acti-Kare policies and quality standards
Caregiver & Field Support
- Conduct caregiver introductions when appropriate
- Review care plans with caregivers and make sure expectations are clearly understood before care begins
- Provide hands-on coaching and skills reinforcement when caregivers need additional support with a client's specific care needs
- Help caregivers problem-solve challenges they encounter in the home
- Recognize when a care issue falls outside caregiver scope or requires escalation to a nurse or other clinical professional
- Reinforce quality standards, documentation expectations, and best practices in the field
- Provide occasional emergency field shift coverage when a scheduled caregiver cannot report and no backup caregiver is available
- Participate in a rotating on-call schedule (with support), responding to urgent client, caregiver, and scheduling needs outside normal office hours

Required Qualifications
- Active Washington State CNA or HCA credential in good standing
- At least 1 year of hands-on caregiving or patient care experience
- At least 1 year of professional client-facing experience in home care, senior care, healthcare, case management, patient services, community health, social services, or a related field
- Natural ability to build trust with people during stressful or emotional situations
- Professional presence and communication style appropriate to an in-home setting
- Strong verbal and written communication skills
- Strong organizational skills and exceptional follow-through
- Ability to manage multiple priorities without losing important details
- Ability to work independently and exercise sound judgment in the field
- Valid Washington State driver's license
- Reliable personal transportation and current auto insurance
- Willingness to travel throughout our service area
- Ability to pass required criminal background checks
- Proficiency with Microsoft Office and willingness to learn our care management software
- Willingness and ability to participate in a rotating weekly after-hours on-call schedule
Especially Valuable Experience

You do not need to have done every part of this job before. We are open to someone with strong foundational experience who has the judgment, professionalism, initiative, and ability to grow into the role.
