About Provital Senior Placement & Care Solutions

Provital provides personalized consulting and advisory services to help seniors and their families navigate the transition to appropriate living and care solutions. We work closely with families to understand their unique needs and guide them toward the ideal options across independent living, assisted living, memory care, and home care.

Senior Placement Case Manager

The Senior Placement Case Manager serves as a trusted advisor to older adults and their families, helping them navigate the often-complex decisions surrounding senior living, home care, memory care, and long-term care options. Through personalized assessments, education, advocacy, and ongoing support, the consultant matches clients with solutions that best align with their care needs, lifestyle preferences, financial considerations, and personal goals.

Key Responsibilities

Meet with seniors and their families to assess care needs, lifestyle preferences, health concerns, and financial considerations.
Educate clients about available senior living, home care, and long-term care resources.
Research and identify suitable independent living, assisted living, memory care, and home care options.
Coordinate and schedule tours of senior living communities.
Facilitate introductions between clients, families, and care providers.
Assist families in comparing communities, services, costs, and care offerings.
Support clients throughout the transition process, including move-in coordination when needed.
Serve as a neutral advocate focused on the client's best interests.
Provide emotional support and guidance during difficult decision-making processes.
Maintain regular communication with clients and family members throughout engagements.
Track client interactions within the system and maintain accurate case records.
Ensure compliance with all applicable privacy and confidentiality standards.

Preferred Experience

Experience maintaining relationships with hospitals, rehabilitation centers, home health agencies, senior living communities, and community organizations.
Participate in networking events, educational programs, and industry conferences, as needed.
Stay informed about industry trends, regulations, and best practices in senior care.

Ability to manage multiple client cases simultaneously

Qualifications

Education in Social Work, Customer Relationship Management, Healthcare Administration, Nursing, or a related field preferred.
Experience in senior living, healthcare, social work, case management, home care, or related fields.
Strong understanding of the senior care continuum.
Excellent interpersonal, communication, and presentation skills.
Ability to build trust with seniors, families, and partners.
Strong organizational and time-management skills.
Ability to navigate complex family dynamics with professionalism and empathy.

Work Environment

Note that this is a work from home position which requires in-field work to meet with clients etc.
The candidate is expected to be able to visit clients etc. within a 20-mile radius of Burlington, MA.
The candidate must have a reliable source of transportation.
