Full Job Description

Los Angeles Centers for Alcohol and Drug Abuse (LA CADA)

Position: Community Health Worker

Schedule: Monday -Friday 8:00 am - 5:00 pm

Summary: Under the supervision of the Program Manager, the Community Health Worker is responsible for providing outreach, complete referral process, assessment, and client enrollment, coaching and social support, and care coordination and navigation services to justice-involved individuals.

Essential Job Functions:

Assist Program Manager with daily operations, including but not limited to, tracking, data analysis and reports.
Assist with administrative responsibilities, including but not limited to, employee timekeeping, mileage reimbursement, invoicing and other required administrative responsibilities as assigned.
Receive participant referrals directly from County jails, Probation Department, and the community with the objective to enroll individuals into the Reentry Intensive Case Management program.
Conduct assessment of participant’s strengths and needs by utilizing appropriate screening and/or assessment tools.
Develop a care plan with the participant’s own goals, provide intensive case management and service navigation such as referral and linkage to physical health, mental health, and substance use disorder treatment services; housing support services; employment and education services; and other supportive services tailored specifically for the client.
Obtain consent to exchange communication with referring entity on the individual’s service needs and reentry case plan.
Complete all necessary and required documentation, which includes the use of Comprehensive Health Accompaniment Management Platform (CHAMP).
Model advocacy and support while empowering participant to advocate for self to achieve health, life, and service plan goals.
Conduct in-person meetings on a weekly or bi-weekly basis and reassess participants every month as evidenced by CHAMP documentation.
Follow up with participants to encourage engagement, ongoing participation, and commitment to the program.
Attend all required trainings and events organized by the County to foster learning and community.
Maintain current knowledge of local case management resources.
Refer clients who are high risk for HIV/STDs to testing services and refer high-risk HIV negative clients to pre-exposure prophylaxis counseling services.
Assist the development of project outreach materials.
Participate in meetings of the project Community Advisory Committee, to provide insight to the strategies and challenges with outreach, service delivery and participant retention.
Attend related community meetings and task forces.
Work with Project Manager and the project evaluation team to collect and report client referral data.
Maintain participant confidentiality and privacy by protecting participant health information, in compliance with HIPAA, and State, Federal and County regulations.
Assure that all County guidelines and criteria are met.
Adhere to Culturally and Linguistically Appropriate Services (CLAS) standards.
Other duties as assigned by the Program Manager

Qualifications:

Bachelor’s degree in related field preferred with a minimum of two years profession or lived experience working with the project population in the delivery of intensive case management services.
Knowledge of prevention strategies related to substance use disorders and HIV/AIDS are a plus. Should have a history of incarceration and have demonstrated a period of stability and successful reintegration into the community following incarceration. Prior experience working with individuals with system involvement, chronic medical issues, mental illness, SUD, or experiencing homelessness preferred and/or Experience assisting community members in navigating health and social service system. Minimum three years of administrative experience in behavioral health services.
Knowledge of integrated behavioral health services.
Must be computer literate with excellent verbal and written communication skills.
Not currently on probation or parole. Must be discharged 2 years.
Must pass background check; and fingerprint clearance (DOJ).
Valid California Driver’s License and proof of insurability.
Bilingual English/Spanish optional.
Must maintain CPR / First Aide Certification, and provide yearly physical exam, including Tuberculosis test results.
Bilingual English/Spanish preferred, but not required.

Work Environment and Physical Demands

The Community Health Worker works in an office environment as well as in various community settings as required by the program demands.
The Community Health Worker may be required to travel locally for meetings, trainings, conferences, or site visits.
The Community Health Worker may be exposed to stressful or traumatic situations when working with participants or communities affected by substance use disorder, mental health issues, incarceration, reentry, trauma, violence, or other challenges.
While performing the duties of this job, the employee may work in an office environment, workstation, shared space, or program location.
Some outdoor activities may be required.
The ability to work in a fast-paced environment.
Occasional lifting more than 10 lbs
Sitting, standing, walking, and reaching are performed in the normal course of a shift.
The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee engaged in potential exposure.
The noise level in the work environment is occasionally moderate with occasional high volume.

Must be able to pass the following pre-employment screenings:

Background check, including criminal history (LA CADA considers all applicants equally and will take into account each applicant’s individual life circumstances).
Drug and health screen, including negative TB test

Additionally, L.A. CADA offers HIV / AIDS prevention and intervention services, including HIV testing.
