Full Job Description

Case Manager- Enhanced Care Management - Bilingual Chinese

Garfield Health Center is seeking a full time Chinese-speaking Case Manager for our El Monte location. Individual must possess a strong work ethic background and have a great positive attitude. We would like someone that enjoys jumping in with the team and wants to be part of a team player environment.

Primary/Essential Duties and Responsibilities:

· Introduce and enroll eligible population to the Enhanced Care Management Program

· Work with the Enhanced Care Management members who has multiple chronic conditions, high utilization of the ER and hospital and/or history of chronic homelessness

· Educate the members with multiple chronic conditions about evidence-based standards of care and self-management of their illnesses

· Collaborate with primary care providers and behavioral health providers to provide health and behavioral interventions that will maximize the Health Homes Program members' health outcomes

· Conduct assessments of the members' history with medical/dental/behavioral health services, social and economic resources for purposes of developing a comprehensive care plan and providing linkage to services

· Document work through appropriate record keeping that follows GHC’s policies and procedures

· Links the members to needed services and community resources

· Work with members and staff from other entities to coordinate discharge details for patients exiting care facilities (hospital, ED, treatment centers)

· Monitor the members' treatment adherence

· Offer services where patients live or congregate in order to increase access to care.

· Facilitate integrated/collaborative case coordination meetings; participate in monthly administration/evaluation meetings

· Attend appropriate community resource meetings/trainings, as assigned

· Attend meetings with health plans- shared success story and presenting cases to health plan

· Updating the monthly telephone list from the health plans to track member's activities and being the assigned case manager to conduct the final review for the telephone list before submission to the health plans

· Provide care coordination to patients assigned from clinical providers, connecting them to health care, psychosocial support, and other services

Work Schedule/ Location:

Full time: 40 hours per week
Schedule: You get to choose a fixed work schedule of either Mon-Fri (8:30am to 5:30pm, 1 hour lunch) in El Monte or Tues-Sat (Saturday from 8:30am to 5pm, 30 minute lunch, in Monterey Park).

Requirements:

Bilingual in Chinese (Cantonese or Mandarin)- required
ECM experience- strongly preferred
Social service or Adult Day Health Care experience- strongly preferred
Bachelor's Degree (minimum level of education)- required
