Job Overview

Join our compassionate team as a Home Care Aide (HHA) and make a meaningful difference in the lives of individuals requiring personalized in-home care. In this vital role, you will provide essential support to clients, helping them maintain independence, comfort, and dignity. Your energetic and caring approach will foster a positive environment, ensuring each client receives attentive, respectful, and professional care tailored to their unique needs. Duties

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patient health by observing vital signs such as blood pressure, pulse, and temperature; report changes promptly
Administer medications accurately following medical instructions and maintain detailed medical documentation
Support clients with meal preparation, light housekeeping, laundry, and maintaining a clean living environment
Use patient transfer methods safely, including Hoyer lifts and other assistive devices, ensuring client safety at all times
Document care activities in electronic health record (EHR) or electronic medical record (EMR) systems while maintaining strict confidentiality in accordance with HIPAA regulations
Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory-related conditions; assist with end-of-life care when necessary
Follow home healthcare regulations and protocols to ensure compliance during all caregiving activities
Communicate effectively with clients and their families to address needs and preferences while fostering trust and comfort

Skills

Proven experience working with individuals with disabilities or in assisted living environments; geriatric or hospice care experience preferred
Strong knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring
Familiarity with home health care assistance tools such as Hoyer lifts and patient transfer methods
Ability to perform heavy lifting safely and efficiently when assisting clients with mobility or transfers
Excellent patient observation skills to identify changes in condition or behavior promptly
Experience working with EMR/EHR systems for accurate documentation of care activities
Knowledge of home health care regulations, HIPAA compliance, and patient confidentiality standards
Effective communication skills for engaging with patients, families, and healthcare teams; compassionate caregiving approach
Basic life support (BLS), first aid, and CPR certification to respond effectively in emergencies
Experience working with individuals with developmental disabilities or dementia enhances your ability to deliver personalized care
