POSITION SUMMARY:

The Community Inclusion specialist/ Job Coach position assists individuals with varying disabilities to build meaningful relationships, Volunteer, and Maintain employment in typical integrated settings of the individuals choosing with other community members with similar interests. THis position will be primarily Community Inclusion with some job coaching.

Description of Community Inclusion

The Department of Disabilities Administration defines community inclusion as follows.

“Community Inclusion is an individualized service that provides individuals with opportunities to engage in community based activities that support socialization, education, recreation and personal development for the purpose of: building and strengthening relationships with others in the local community who are not paid to be with the person and learning, practicing and applying skills that promote greater independence and inclusion in their community.”

EDUCATION and/or EXPERIENCE:

· High school diploma or general education degree (GED) required.

QUALIFICATIONS:

· Must be at least 21 years of age.

· Must have the ability to satisfy a background check and comply with the company's drug free workplace policy.

· Must provide proof of current driver’s license and insurance.

· Must secure and maintain First aid and CPR certification within 30 days of employment.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following

Operational

· Develop an individualized plan of service with each person supported and maintain knowledge of their community inclusion goals and support needs.

· Participates as part of individual’s support team by attending annual meetings conducted by DDA

· Provides feedback to the Manager about any changes that may be needed in an individual’s service plan to help them better attain their goals.

· Provide in person services in the community to each person on ones case load at least one time each week in accordance to their Community Inclusion Plan/ Employment Plan.

· Maintains an accurate and detailed record of services delivered, and time worked,

· Submits all documentation in a timely manner (Dialy).

· Attends and participates in scheduled staff meetings and completes required training.

· Create your schedule to satisfy hours assigned to you and each person served

· Maintains positive and professional relationships and good communication with co-workers, supervisor, and all other employees and Individuals supported at True North Employment Solutions.

Provide on the job support to individuals. Which may include visual and Physical demonstration, Verbal Cuing and more as needed.

· Complete other Duties as assigned.

· Follow dress code guidelines to provide an appropriate role model and to represent quality services.

· Act according to the guidelines of the company policies. Maintain working knowledge of these documents by reviewing them regularly.

· Stay calm in challenging situations, maintains positive attitude, and serve as a role model for individuals served and employees.

· Maintains a safe working environment, follows safe work practices, and contributes to overall team safety as defined in the employment services manual.

· Quiet to moderate noise level in work environment and often required to sit or stand for long periods of time

· Required to talk, hear and the ability for close vision.

· Often required to use hands and fingers to handle, feel or to manipulate keys on a keyboard.

· Occasionally required to stand, walk, reach with arms and hands, climb/balance, stoop, kneel or crouch.

· Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.

· The employee must occasionally lift and/or move up to 25 pounds.
