Full Job Description

Dear Applicants,

Sea Coast Hospice is looking for a registered nurse for admissions and supervisory visits in San Diego County. This is great for a nurse who needs flexibility and will work with the hospice agency to provide a mutually agreeable work schedule. Many of our patients are located in the same facility which helps reduce our staff travel time. If you are a warm, caring nurse who is passionate about hospice care then we would love to hear from you!

Responsibilities:

Completes patient and family assessments to determine hospice needs
Provides skilled nursing care for pain and symptom management
Provides emotional support and counseling to patients and their families with the support of the interdisciplinary team
Creates an individualized plan of care tailored to patient and family/caregiver needs
Administers medications and treatments as prescribed by a physician
Provides health care instructions to the patient and family/caregiver as appropriate per assessment and plan of care
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers
Works with an interdisciplinary team to facilitate providing care that addresses the physical, psychosocial, and spiritual needs of patients and family/caregivers

Qualifications:

Registered Nurse with current unrestricted CA license
Excellent observation, verbal and written communication skills, and problem-solving skills.
Must be a licensed driver with an automobile that is insured by state/organization requirements and is in good working order
Possesses and maintains current CPR certification.

Job Type: Full-time
