ESSENTIAL functions AND RESPONSIBILITIES:
1. Supervises multiple Direct Support Professionals (DSPs) who provide direct care services to
individuals served within the statewide CES Waiver program. Manages and oversees service
delivery by DSPs for all individuals assigned to caseload. Maintains an assigned caseload of
consumers while ensuring quality, continuity, and compliance of services. Monitors staff hours and days worked in comparison to the approved Plan of Care and communicates any necessary adjustments with the individual and/or representative.
2. Conducts interviews and participates in the hiring and selection process for Direct Support Professionals (DSPs) assigned to individuals on their caseload. Assists with staffing coordination by helping identify shift coverage needs, locating available staff, and providing support with coverage arrangements when DSP call-ins or staffing shortages occur to help ensure continuity of care and services. Remains on-call for after-hours emergencies, crisis intervention, or urgent service needs for individuals on caseload, as assigned through the established on-call schedule.
3. Coordinates and facilitates consumer-specific, person-centered training for newly hired DSPs to help ensure staff are properly trained to meet the individual needs, preferences, routines, and support requirements of each person served. Coordinates staffing schedules for CES Waiver supports, Medicaid services, and other authorized supports and services in a timely and effective manner. Coordinates and facilitates annual client-specific training for assigned DSPs to ensure competency and compliance with individual support needs.
4. Maintains accurate and thorough knowledge of current CES Waiver services, licensing regulations, PASSE requirements, and ACA policies and procedures. Submits prior authorization documentation to the Regional Manager for PASSE approval within established timelines and regulatory requirements. Provides timely written explanations to the Regional Manager regarding underutilization or overutilization of authorized support hours and transportation services, including recommendations for revisions.
5. Schedules and participates in annual ITP meetings with the individual served, legal representative, Care Coordinator, staff, and other requested participants Coordinates the development of annual comprehensive Individual Treatment Plans (ITPs), including measurable goals, staffing needs, transportation, specialized support, housing options, and other identified needs based on the preferences and needs of the individual and/or legal representative.
6. Reviews daily service documentation submitted by DSPs to ensure services are delivered appropriately and in alignment with the goals and objectives outlined in the individual’s plan. Provides coaching, guidance, and feedback to DSPs regarding documentation, service delivery, and performance expectations as needed. Maintains monthly documentation regarding progress, regression, or maintenance of goals identified within the individual’s ITP. Reviews ITP goals, objectives, and outcomes at least quarterly to evaluate effectiveness of services and progress toward identified goals.
7. Ensures the integrity and compliance of all direct service billing by verifying services are authorized, meet required service definitions, align with ITP goals and objectives, and are delivered prior to billing. Maintains case files and documentation for individuals served in accordance with organizational, regulatory, accreditation, and licensing standards.
8. Ensures all client records, tracking systems, and required documentation remain accurate, current, and audit ready. Assists individuals served and, when applicable, parents/guardians with crisis resolution and problem-solving Supports individuals in person-centered service planning (PCSP), choice-making, and self-advocacy efforts.
9. Regularly reviews records, medication documentation, and service environments for individuals served in accordance with departmental expectations and compliance requirements. Conducts and documents quality assurance reviews of service delivery in accordance with departmental expectations and promptly reports any identified concerns or compliance issues to leadership.
10. SUPERVSORY RESPONSIBILITIES: Leads and provides overall direction, coordination, and evaluation of DSPs on their caseload. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training; planning, assigning, and directing work; conducting performance evaluations; rewarding and disciplining employees; addressing complaints and resolving problems.

QUALIFICATION REQUIREMENTS:

Education
Bachelor's degree (B.A.) preferred from a four-year college or university in Psychology, Social Work, Special Education or related field or a minimum of 2 years equivalent experience working with individuals with developmental disabilities in a HCBS or DD setting.

Employees have access to all client protected health information within their cohort of individuals served. Employees will not attempt to access any other protected health information unless otherwise approved by a supervisor or Privacy Officer.
Language Skills
Ability to read and understand basic Agency and DHS regulations, basic medical information and forms, written directions and training plans. Ability to assess individual progress, monitor evaluation reports and case notes, and communicate effectively with individual, other workers of the public.

Pay: $35,000.00 - $45,000.00 per year
