At Chronic Care Link, we partner with physician practices to extend care beyond the office through Chronic Care Management (CCM) and Remote Patient Monitoring (RPM). Our team supports patients between visits, helping improve health outcomes while reducing the administrative burden on providers.

We are building a collaborative, patient-centered care team that believes meaningful relationships, proactive outreach, and exceptional service lead to better outcomes. As a Patient Care Coordinator, you will play an essential role in supporting patients throughout their care journey while collaborating closely with providers and fellow team members in a fully remote environment.

Position Summary

The Patient Care Coordinator serves as the primary point of contact for patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role is responsible for supporting patients throughout their care journey, from enrollment and onboarding through ongoing engagement and care coordination. The ideal candidate is organized, compassionate, technology proficient, and thrives in a collaborative, fast-paced remote environment. As a growing organization, team members are expected to work across multiple operational functions while delivering an exceptional patient experience. Success in this role requires adaptability, accountability, and a willingness to work across multiple operational functions as business needs evolve.

Patient Care Responsibilities

· Serve as a primary point of contact for assigned patients, addressing non-clinical questions and facilitating communication between patients, providers, caregivers, and internal care teams.

· Coordinate patient enrollment into CCM and RPM programs, ensuring a seamless onboarding experience through education, consent, scheduling, and program orientation.

· Conduct high-volume outbound and inbound patient telephone outreach while building trusted relationships through compassionate, professional, and effective communication.

· Build and maintain ongoing relationships with patients through proactive outreach designed to improve engagement, adherence, and overall health outcomes.

· Execute ongoing care coordination activities in support of CCM and RPM program requirements while maintaining high standards of patient service and documentation accuracy.

· Monitor assigned work queues, prioritize daily activities, and ensure timely completion of scheduled patient outreach and follow-up activities.

· Support a collaborative, cross-functional care delivery model by participating in cross-training initiatives and contributing across multiple operational workflows as business needs evolve.

· Collaborate with physicians, practice staff, and internal team members to ensure continuity of care and timely resolution of patient needs.

· Meet established productivity, quality, documentation, and patient engagement standards while contributing to overall team performance.

· Identify barriers to care, patient concerns, or changes in condition requiring clinical review and escalate appropriately through established workflows.

· Support continuous process improvement initiatives while maintaining flexibility to perform responsibilities across multiple operational functions in a cross-trained team environment.

· Protect patient privacy by maintaining compliance with HIPAA and organizational confidentiality standards.

Required Qualifications

· High school diploma or equivalent required.

· 2+ years of healthcare customer service, care coordination, medical assistant, patient support services, care management, or related healthcare experience.

· Strong verbal and written communication skills.

· Experience using electronic health records (EHR/EMR) systems.

· Strong organizational skills with the ability to prioritize competing responsibilities in a fast-paced remote environment.

· Proficiency with Microsoft Office, web-based applications, and the ability to quickly learn new healthcare technology platforms.

· Ability to manage multiple priorities while maintaining attention to detail.

Preferred Qualifications

· Current or previous Medical Assistant experience; CMA or RMA certification preferred but not required.

· Experience with CCM and/or RPM.

· Experience working with Medicare patients.

· Bilingual (Spanish/English).

· Call center or high-volume patient outreach experience.

Remote Work Requirements

· Ability to work independently in a fully remote environment while maintaining productivity, professionalism, and schedule adherence.

· Reliable high-speed internet connection. Verification of internet speed may be required during the hiring process.

Compensation & Schedule

· Competitive hourly compensation of $22.00–$26.00 per hour, based on experience and qualifications.

· Part-time position (approximately 20–30 hours per week) with the opportunity to transition to full-time as the organization continues to grow.

· Monday through Friday during standard Eastern Time business hours. Specific schedules will be determined based on business needs.

· Fully remote within the United States.
