Full Job Description

$4,500 Sign On Bonus, - Monday to Friday, 8am - 4:30pm

We are seeking a dedicated and compassionate Hospice Registered Nurse Case Manager to provide comprehensive end-of-life care and support to patients and their families. The ideal candidate will possess extensive clinical expertise, including critical care, ICU, emergency medicine, and post-acute care experience, combined with a strong understanding of hospice principles and patient-centered care. As a vital member of our healthcare team, the Hospice RN Case Manager will coordinate patient care plans, facilitate communication among multidisciplinary teams, and ensure adherence to healthcare regulations while delivering empathetic support during challenging times.

Duties

Conduct thorough patient assessments, including vital signs, physiological status, and medical history, to develop individualized hospice care plans.
Manage clinical case management activities by coordinating services across inpatient, outpatient, home health, and post-acute settings.
Collaborate with healthcare professionals such as physicians, social workers, therapists, and discharge planners to optimize patient outcomes.
Oversee discharge planning and transition processes to ensure seamless continuity of care in compliance with health regulation policies.
Utilize electronic health records (EHR/EHR management) effectively for documentation, medical records management, and communication.
Educate patients and families on hospice services, symptom management, medication administration, and advance directives with clear client communication.
Monitor patient progress through diagnostic evaluation and case management strategies while managing complex cases involving infectious diseases or behavioral health needs.
Ensure compliance with HIPAA regulations to protect patient confidentiality and uphold high standards of patient service.

Experience

Proven clinical experience in critical care environments such as ICU, Level I Trauma Centers, or Level II Trauma Centers.
Extensive background in hospital medicine, emergency room settings, or medical-surgical units with a focus on acute care.
Demonstrated expertise in post-acute care management including home health and outpatient clinics.
Experience managing patients with diverse needs such as geriatrics, pediatrics (PICU), behavioral health issues, or infectious diseases.
Strong knowledge of health regulation policies related to hospice care and managed care practices.
Proficiency in nursing practices including patient assessment, vital signs monitoring, diagnostic evaluation, and discharge planning.
Familiarity with electronic health records (EHR), medical terminology, HIPAA compliance, and utilization management processes.
Ability to collaborate effectively with multidisciplinary teams to deliver comprehensive patient-centered care across inpatient and outpatient settings.
