We are seeking an experienced, detail-oriented MDS Nurse (RN) to join our Skilled Nursing Facility's clinical leadership team. In this key role, you will oversee and coordinate the Resident Assessment Instrument (RAI) process, ensuring accurate assessments, regulatory compliance, quality resident care, and optimal reimbursement outcomes.

Responsibilities

Coordinate, complete, and submit MDS assessments in accordance with federal and state regulations
Develop, implement, and update comprehensive resident care plans in collaboration with the interdisciplinary team
Ensure timely and accurate completion of all MDS documentation and submissions
Review and validate clinical documentation to support regulatory compliance and reimbursement accuracy
Monitor Medicare, Medicaid, and PDPM reimbursement processes
Maintain compliance with CMS guidelines, RAI regulations, and facility policies
Identify opportunities to improve documentation practices and clinical outcomes
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Collaborate effectively with nursing staff, therapy departments, physicians, and facility leadership
Serve as a resource for staff regarding MDS, care planning, and regulatory requirements

QualificationsRequired

Current Registered Nurse (RN) license in good standing
Minimum of one year of MDS experience in a Skilled Nursing Facility or Long-Term Care setting
Strong knowledge of MDS 3.0, RAI process, CMS regulations, and PDPM
Excellent assessment, documentation, and organizational skills
Strong attention to detail and analytical abilities
Ability to manage multiple priorities while meeting deadlines
Effective communication and collaboration skills

Preferred

RAC-CT certification or willingness to obtain certification
Experience with Medicare and Medicaid reimbursement processes
Prior participation in QAPI or quality improvement initiatives
