WE ARE LOOKING FOR HOSPICE REGISTERED NURSE CASE MANAGER

Pay Range: $45.00 - $50.00 Per Visit Depending on Experience.

Job Overview
Join our dedicated healthcare team as a Home Health and Hospice Registered Nurse - Case Manager, where you will play a vital role in delivering compassionate, patient-centered care across diverse settings. In this dynamic position, you will coordinate and oversee comprehensive nursing services for patients in their homes and hospice environments, ensuring high-quality care that promotes comfort, independence, and dignity. Your expertise will drive effective case management, discharge planning, and clinical assessments, making a meaningful difference in patients’ lives while advancing your professional growth.

Duties

Conduct thorough patient assessments to evaluate health status, vital signs, and physiological needs, utilizing medical terminology and diagnostic evaluation skills.
Develop personalized care plans aligned with patient goals, medical records, and HIPAA regulations to ensure privacy and compliance.
Manage inpatient and outpatient caseloads by coordinating post-acute care, discharge planning, and utilization management to optimize patient outcomes.
Provide skilled nursing interventions across various environments including hospital medicine units, ICU, PICU (Pediatric Intensive Care Unit), behavioral health facilities, and outpatient clinics.
Collaborate with interdisciplinary teams to facilitate diagnostic evaluations, manage infectious diseases, and address complex cases involving geriatrics or pediatric populations.
Oversee intake processes for new patients, ensuring proper documentation of medical records and adherence to hospital experience standards.
Educate patients and families on disease management, medication administration, and health maintenance strategies to promote self-care at home or in hospice settings.
Maintain accurate medical records and ensure compliance with all regulatory standards including HIPAA and hospital policies.
Participate in case management activities such as utilization review and coordination of outpatient services to support continuity of care.

Experience

Proven experience in critical care environments such as ICU or Level II trauma centers with exposure to Level I Trauma Center protocols preferred.
Extensive background in hospital medicine or emergency room settings demonstrating proficiency in patient assessment and acute care management.
Prior involvement in post-acute care settings including home health or hospice nursing with a focus on discharge planning and outpatient services.
Familiarity with managed care practices, medical-surgical nursing, behavioral health considerations, and inpatient/outpatient clinic operations.
Strong knowledge of physiology principles related to diverse patient populations including pediatrics (PICU), geriatrics, and patients with infectious diseases or complex chronic conditions.
Experience utilizing diagnostic tools for evaluation purposes and managing patient service workflows efficiently within hospital or outpatient environments.
Demonstrated ability to perform comprehensive patient intake assessments while maintaining compliance with HIPAA regulations.
Ability to adapt nursing skills across multiple specialties such as OR experience or emergency medicine to provide versatile patient care solutions.
