Job Overview

Join our dynamic team as a Health and Wellness Director, where your leadership will shape the quality of life for residents in long-term care environments. This vital role combines clinical expertise with compassionate management to ensure residents receive exceptional care tailored to their individual needs. As a key driver of regulatory compliance, staff supervision, and care excellence, you will foster a positive, resident-centered atmosphere that promotes health, safety, and well-being. Your energetic approach and dedication will inspire teams to deliver top-tier services while maintaining the highest standards of healthcare standards and regulations.

Responsibilities

Lead and oversee the planning, implementation, and evaluation of comprehensive care programs aligned with residents’ needs and regulatory requirements.
Supervise nursing staff, including nurse managers and care coordinators, ensuring effective shift management and high-quality patient care delivery.
Ensure compliance with all long-term care regulations, HIPAA privacy standards, and state licensing requirements.
Manage documentation review processes, including medical records, care plans, MDS (Minimum Data Set) assessments, and medical terminology accuracy within EMR (Electronic Medical Records) systems.
Promote resident rights by fostering an environment of dignity, respect, and personalized care; educate staff on resident’s rights policies.
Oversee clinical supervision activities such as staff training on medical records management, documentation review, and adherence to HIPAA regulations.
Collaborate with interdisciplinary teams to develop tailored care plans that incorporate physiology and anatomy knowledge for optimal health outcomes.
Drive quality improvement initiatives by analyzing patient care data and ensuring regulatory compliance across all clinical operations.
Manage home health services or hospice care programs when applicable, ensuring seamless coordination of services for residents requiring specialized support.

Requirements

LPN/RN License
Proven experience in assisted living facilities, nursing homes, or long-term care environments with a strong understanding of long-term care regulations.
Demonstrated leadership skills with supervising experience in nursing administration or nurse management roles.
Extensive knowledge of medical terminology, physiology, anatomy, and patient care practices within acute or chronic care settings.
Familiarity with EMR/EHR (Electronic Health Records) systems for documentation review and medical records management.
Experience managing care facility teams with a focus on shift management and administrative clinical supervision.
Strong understanding of Medicare regulations, regulatory compliance standards, and long-term care facility operations.
Excellent communication skills to effectively lead staff meetings, training sessions, and resident advocacy initiatives.
Ability to navigate HIPAA privacy rules while managing sensitive patient information securely.
