Full Job Description

MISSION:

Our mission, grounded in traditional Native American culture, is to reduce health disparities by delivering high-quality, comprehensive integrated healthcare and providing stable housing throughout the communities we serve.

POSITION SUMMARY:

Under the direct supervision of the Clinical Director this position is responsible for completing all client assessments in a timely manner and recommending clinically appropriate care based on the outcome of the assessment. Placement may be in a Native American Connections (NAC) program or with another agency who can provide the appropriate level of care.

RESPONSIBILITIES:

Complete all assessments and related documentation. Recommends appropriate services for client based on assessment outcome. Make Referrals to internal programs or to external providers as appropriate based on assessment.
Complete updated Assessment and ASAM as needed.
Complete Individual Service Plan in collaboration with client.
Assists with case management as needed.
Works closely with the admissions team to prioritize (i.e., Native American, pregnant, IV substance use, homeless, etc.) residential admissions.
Notify clients and referral sources of all documents and requirements for entering treatment prior to arrival
All documentation from assessments and/or client interactions will be completed and available in the electronic health record (EHR) within 24 hours.
Maintain positive working and referral relationships with T/RBHAs, Phoenix Area Indian Health Service offices and tribes, detox centers, Phoenix Indian Medical Center, medical facilities and other numerous referral agencies.
Assist with conducting group or individual counseling to clients as outlined in clinical curriculum as needed.
Support and actively engage in practices of inclusion through Cultural and Linguistically Appropriate Services (CLAS) Standards.
Attends regular clinical supervision and oversight meetings.
Other duties as assigned.

TIER II AND TIER III ADDITIONAL RESPONSIBILITIES MAY INCLUDE:

Review and sign off on intake assessment, ASAM, and treatment plan for non-licensed staff
Review of clinical documentation as required by licensing agencies
Consultation and recommendation om client appropriateness for care
Assist with onboarding and training of new staff

EDUCATIONAL /WORK EXPERIENCE REQUIREMENT:

Tier I ($55,000 - $64,000)

o Bachelor’s degree in behavioral health field required

o 2 years’ experience working in behavioral health or substance abuse preferred

Tier II ($65,500 - $71,500)

o Master’s degree in behavioral health field required

o Associate level licensure in a behavioral health field (LMSW, LAC, LAMFT, or LAAC)

o 2 years’ experience working in behavioral health or substance abuse preferred

Tier III ($70,000 - $91,000)

o Independent licensure in a behavioral health field (LCSW, LPC, LMFT, LIAC)

o 2 years’ experience working in behavioral health or substance abuse preferred

SKILLS REQUIREMENT:

Fingerprint Certificates within 90 days of hire.
MS Office proficiency
Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
Demonstrate excellent written and oral communication skills.
Must demonstrate critical thinking, problem- solving and organizational and time management skills.
Bilingual- Spanish desired
Required strong clinical skills.
Knowledgeable about the economic, educational, and social challenges of Native Americans
Familiar with assessment tools that pertain to substance abuse, depression, and other behavioral health issues.
Familiar with the DSM (most recent revision) and diagnostic criteria.

PHYSICAL DEMANDS:

Able to sit, stand walk with/ without accommodations.
