Full Job Description

JOB SUMMARY

Under the Supervision of the Director of Child & Adolescent Services this position is responsible for performing assessments, including crisis assessments as needed, developing recovery plans, discharge planning, along with providing interventions to youth individually and in groups and may assist in day programming activities at respite for youth ages 13 to 17years old. Ability to communicate with youth and their families to promote collaboration when creating recovery plans. This role will also include collaboration with contract providers and clinical teams. This role requires assistance and support with respite admissions. Must be a self motivator, able to work independently and remain flexible to solve problems and crises. In-region travel is required as needed. Out of region travel may be required occasionally for training. Services will be primarily at youth crisis respite facility but at times it may be in the local clinic to meet the needs of those youth being served. Voluntary on call during evening and weekend hours available. Must be familiar with, and/or willing to be trained in, trauma-informed care. This includes understanding, recognizing, and responding to the effects of all types of traumas, and an ability to work in a behavioral environment where staff convey dignity, respect, hopefulness, and the opportunity for choice and empowerment among consumers.

DUTIES & ESSENTIAL JOB FUNCTIONS

Provides case management services and rehabilitation services for youth with mental illness (ages 13 to 17 years). Coordinates and provides services. Demonstrates a positive role model to youth and advocates on the family’s behalf. Responsible for coordination, planning and implementation of the recovery plan and discharge plan. Duties include but are not limited to the following:

Maintain understanding of TRR services and UM Guidelines for assigned levels of care.

Accurately complete a uniform assessment to identify an individual’s needs and strengths as appropriate.

Must be able to utilize an electronic health record system to document collaboratively and concurrently with consumers across all processes, including assessment, service planning, and ongoing consumer provider interactions.

Meet productivity standards set by the agency.

Provide community referrals as needed.

Communicate with referring entities, internal and external providers.

Maintain a flexible work schedule while demonstrating effective time management to meet needs of individuals and families served.

Travel between locations assigned.

Participate in monthly Youth Services staff meetings.

Participate in data collection activities and peer review processes as requested.

Plan and facilitate groups at respite

Occasionally assist with other youth programs outside of respite program if needed.

Other duties as assigned by supervisor.

KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of youth issues in a rural setting. Ability to work with schools, juvenile probation and other social service agencies to facilitate services for youth. Ability to communicate with youth and their families to promote collaboration in recovery plan development. Work as a team member and independently while remaining flexible to solve problems and crises. Additional knowledge, skills, and abilities include:

Working knowledge of mental health and substance abuse services and needs.

Clear understanding of normal and atypical childhood development.

Must have strong computer operating skills.

Ability to participate respectively as a team member during department and agency meetings.

Excellent verbal and written communication skills.

Ability to work under pressure and meet deadlines while maintaining a positive attitude and providing exemplary customer service.

Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.

Capacity for flexibility in dealing with ongoing changes in the service and delivery system.

REQUIRED EDUCATION AND EXPERIENCE

Education: Bachelor’s degree in a human services field from an accredited college or university is required.

Master’s degree preferred.

Experience: Two years direct experience working with children, teens and their families experiencing mental health challenges in a community setting is preferred.

REQUIRED LICENSES/CERTIFICATIONS

No professional license is required. QMHPCS status is necessary

Must have a valid Texas driver’s license, a good driving record for the past three years, and personal automobile liability insurance as required by the state of Texas.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

Must be able to bend, stoop, kneel, crouch, squat, lift 25 pounds, sit for an extended period, and reach overhead.
