Manager, Case Management – (Adult Services)

Location: Houston, TX
Job Type: Full-Time
Salary: $100,000 – $130,000 per year

Position Summary

We are seeking an experienced Manager, Case Management – Trauma (Adult Services) to lead and oversee the daily operations of our adult trauma case management team in Houston, Texas. This leadership role is responsible for ensuring high-quality patient care coordination, effective utilization management, discharge planning, and regulatory compliance while driving positive patient outcomes and operational excellence.

The ideal candidate is a collaborative healthcare leader with strong clinical or social work experience, exceptional communication skills, and a proven ability to lead multidisciplinary teams in an acute care hospital setting.

Key Responsibilities

Lead, mentor, and develop the adult trauma case management team.
Oversee case management, utilization review, discharge planning, and care coordination activities.
Ensure compliance with regulatory, accreditation, and organizational standards.
Collaborate with physicians, nursing leadership, social services, and interdisciplinary teams to optimize patient outcomes.
Monitor quality metrics, length of stay, resource utilization, and patient throughput.
Identify opportunities for process improvement and implement best practices.
Support staff education, performance management, and professional development.
Maintain accurate documentation and ensure adherence to hospital policies and procedures.

Required Qualifications

Current and valid Registered Nurse (RN) license in the State of Texas OR current Licensed Master Social Worker (LMSW) license in Texas.
Licensed Clinical Social Worker (LCSW) preferred.
Case Manager Certification (CCM, ACM, or equivalent) required.
Minimum 5 years of experience in utilization management, case management, discharge planning, or other cost/quality management programs.
Minimum 3 years of hospital-based nursing or social work experience.
Minimum 3 years of demonstrated leadership or management experience.

Preferred Qualifications

Adult trauma or acute care hospital experience.
Strong knowledge of CMS regulations, utilization review, discharge planning, and care coordination.
Experience leading interdisciplinary teams in a fast-paced healthcare environment.
Excellent leadership, organizational, communication, and problem-solving skills.
