Full Job Description

Beverly Glenn, a respected recruiter for two of the largest home health agencies in Southern California, is seeking a compassionate and dedicated Occupational Therapist (OT) to join our growing home health teams. Our partner agencies are licensed, Medicare-certified, and CHAP-accredited, providing exceptional rehabilitative care across Southern California.

This position offers the opportunity to provide meaningful, patient-centered therapy in the comfort of clients’ homes and community settings. The ideal candidate has experience across diverse clinical environments—including pediatrics, geriatrics, acute care, outpatient settings, and post-acute rehabilitation—and is passionate about helping patients regain independence and enhance quality of life.

Education & Licensing

Graduate of an accredited Occupational Therapy program.
Current, active California OT license.
Current CPR/BLS certification (if required by agency).
Valid driver’s license and reliable, insured vehicle that meets state and agency requirements.

Experience & Qualifications

Experience working with diverse patient populations including:
Pediatrics (including early intervention),
Geriatrics,
Individuals with disabilities or autism spectrum disorder,
Acute and post-acute care patients.
Familiarity with a variety of practice settings such as home health, outpatient clinics, skilled nursing facilities, inpatient hospital units (including ICU), and school-based environments.
Strong understanding of physiology, anatomy, medical terminology, and rehabilitation principles.
Ability to independently develop and modify individualized care plans.
Excellent documentation, communication, and critical-thinking skills.
Comfortable using digital documentation systems and technology to streamline care coordination.
Physically able to walk, bend, stoop, and lift up to 50 lbs during patient care tasks.
Self-directed, organized, and adaptable to changing patient needs and schedules.

ESSENTIAL DUTIES & RESPONSIBILITIESPatient Assessment & Care Planning

Conduct comprehensive evaluations of patient functional abilities, safety needs, and rehabilitation goals.
Develop personalized treatment plans focused on improving independence with activities of daily living (ADLs), mobility, cognition, and overall functional performance.
Modify treatment plans as necessary based on patient progress and clinical judgment.

Therapeutic Interventions

Provide direct therapy services in patients’ homes and community settings, supporting ADLs such as dressing, grooming, feeding, toileting, and home safety.
Implement evidence-based interventions for patients across diverse populations, including pediatrics, geriatrics, individuals with developmental disabilities, autism spectrum disorder, and those recovering from injury, surgery, or hospitalization.
Educate patients, families, and caregivers on therapeutic exercises, adaptive strategies, safety precautions, durable medical equipment, and home modifications.

Interdisciplinary Collaboration

Collaborate with families, caregivers, physicians, nurses, case managers, and other rehabilitation professionals to support comprehensive, coordinated care.
Participate in case conferences, interdisciplinary planning sessions, and staff meetings.
Contact physicians to clarify orders or communicate significant changes in patient condition.

Care Coordination & Documentation

Assist with discharge planning and care transitions to ensure continuity across settings such as hospitals, outpatient clinics, schools, or skilled nursing facilities.
Maintain accurate, timely, and compliant documentation including evaluations, treatment notes, care plans, progress summaries, and discharge reports.
Adhere to regulatory standards, agency guidelines, and Medicare home health documentation requirements.

Key Skills

Strong clinical reasoning and assessment abilities.
Ability to adapt therapy approaches across varied environments (home, outpatient, inpatient, schools, SNFs).
Skilled in patient and family education.
Knowledge of occupational health principles related to workplace safety and injury prevention (a plus).
Effective communicator capable of collaborating with multidisciplinary teams.
