We are a Home Health Company based in Tucson, Arizona and are in need of an RN Clinical Service Manager for our Tucson home health agency office. We are looking for an experienced RN with leadership experience to help oversee all nursing operations of the Tucson office.

Core duties and responsibilities include the following (other duties may be assigned).

Plans and organizes activities related to specified home health clinical services to ensure patient needs are met in accordance with professional standards of medical and nursing care and administrative procedures.
Coordinates activities with departments, agencies, and health facilities as needed.
Develops, implements, and evaluates methods to ensure effective and efficient use of staff, facilities, equipment, management, utilization review, and discharge planning programs.
Develops and implements the treatment plan for each patient under the direction of the patient's physician.
Ensure effective communication to coordinate the implementation of each patient's plan of care.
Provides support, guidance, and education to patients and patient's families and/or caregivers to maintain optimal physical and emotional wellbeing.
Completes all clinical documentation in a timely manner, including admissions, recertifications, and resumptions of care accurately and ensures that documentation meets all requirements per agency-policies and state regulations.
Performs bedside nursing duties or other direct patient care when necessary.
Must be willing to work rotating on call shifts.
Must be willing to travel to patient's home for Case Management.
Learn monthly scheduling to create clinician schedules in advance.
Learn the EEIOCPA program and how to write effective Letters of Medical Necessity (LOMN).
Participate in the weekly Policy and Procedure meeting to validate or update policies and procedures as outlined by state and local requirements.
Consults with other case managers regarding patient care or nursing problems and policy interpretation to ensure patient needs are met.
Collaborates with the Administrator/Administrative Director to resolve client/clinical concerns promptly.
Communicates all occurrences and other pertinent information to the Administrator/Administrative Director.
Ensures any observed or suspected adult abuse, neglect, or exploitation is reported to proper authorities as mandated as a health professional.
Participate in interviewing, hiring, training, and supervision of clinical staff and home health aides. Plans and organizes orientation and in-service training for staff members and participates in guidance and educational programs. Participates in agency sponsored continuing education in-service and conferences to maintain own clinical competency. Enforces/demonstrates compliance with State/Federal regulations.
Complies and participates in the continuing quality improvement by appropriately reporting incidents, injuries, infections, falls, and all components of the risk management program.
Assists the Administrative Director and all staff to obtain new patients from various marketing efforts.
Supports and executes the mission, ethics, and goals of the company effectively.
Represents themselves in a positive and professional manner in the company and community. Adhere to dress code with a clean and neat professional appearance. Reports on time and as scheduled in order to complete work within the a designated time.
Adheres to all company policies and procedures outlined in employee handbook, employee agreement, code of business conduct/ethics or communicated from the executive team.
Other duties may be assigned as necessary for the purpose of ensuring the efficient and effective functioning of the work unit.
RN (Required)
3 years of home health experience preferred
Current TB Test and a second test to create a baseline with the agency
BLS Certification (Required)
CPR
Fingerprint Clearance Card

Work Location: In person
