Job Title: Registered Nurse (RN) Case Manager | Home Health Autonomy & Patient Connections
Company: Home Health Care, Inc.
Location: Burnsville, Eagan + 35 mile radius from home.
Schedule: Full-Time | Monday-Friday (8:00 AM - 5:00 PM) + On-Call and Weekend Rotation
Compensation: $85,000-$130,000+ annually + $6,000 First-Year Bonus

What You BringRequired

Active Minnesota RN license
Three or more years of nursing experience
