Full Job Description

Qualifications: High school diploma or equivalent; experience in the human services field working with people with physical and /or intellectual disabilities, the elderly or children and youth preferred. Must have clear Criminal History and Child Abuse checks less than a year old. Must have clear FBI check less than a year old if a non-Pennsylvania resident, anytime during the last two years. Must have a valid driver’s license.

Required knowledge, skills and Abilities:

Ability to develop and maintain sympathetic and understanding attitude when working with individuals.
Ability to show good judgment regarding the health and welfare of individuals and respond appropriately in emergency situations.
Ability to take direction as well as direct/redirect consumers.
Ability to establish and maintain effective working relationships with staff and consumers.
Ability to express ideas clearly and concisely, orally, and in writing.
Knowledge of basic cooking skills and ability to follow a recipe.
Ability to exercise good judgment and discretion.

Essential Job Functions:

Comprehend and complete required documentation.

Carry out goals and objectives in the consumer’s Individual Support Plan (ISP) on a daily basis.

Must successfully complete required training sessions.

Must be able to clearly communicate orally and in writing.

Must be able to interpret and follow written and verbal instructions.

Interact professionally with staff, parents and consumers.

Accomplish personal care duties, housekeeping activities and other related duties.

Must be able to drive and willing to drive passenger van or car.

Duties:

Maintain a professional and courteous attitude at all times
Ensure a safe and secure environment for consumers, self and co-workers.
Report work related accident, injury, illness or near misses to risk/quality manager and/or Assistant Director within reported timelines, writing incident report and faxing the incident report immediately.
Ensure the accurate documentation of everything regarding the consumer with hourly entries during every shift.
Attend and participate in all training and staff meetings regarding the consumer as requested and/or required. Meet 24 hours annual training requirement in addition to agency required annual training such as fire safety, blood borne pathogens, medical administration and drug free work place.
Adhere to confidentiality policies regarding consumer and staff.
Never physically, verbally or psychologically abuse a consumer. Report any suspected abuse to the supervisor or designee immediately.
Provide care guidance for all consumers. Must provide the level of supervision noted in the individualized service plan (ISP) for each consumer.
Ensure that the consumer personal hygiene needs are met every day.
Provide the consumers with the opportunity to participate in the community outside of their specific goals.

Assist the consumer to use appropriate verbalization to express her emotions.
Help the consumer in learning and implementing calming techniques, coping skills and anger management techniques (i.e., relocation, breathing, etc.) to replace, manage and de-escalate emotional outbursts.
Encourage the consumer to discuss other ways of dealing with anger after every incident of aggression. After the consumer calms down, encourage the consumer to talk about the incident. Speak with the consumer in a calm and cool manner.
Help the consumer improve socialization skills to enhance positive social relationship with peers, staff and others.
Encourage the consumer to keep up with all medical appointments as well as taking all medications as prescribed daily.
Make sure the consumer’s medications are refilled when necessary.
Assist the consumer to maintain a healthful environment by encouraging him/her if necessary to perform the following housekeeping tasks for her safety and comfort:
Dusting, sweeping and vacuuming
Routine cleaning of refrigerator and stove
General cleaning of the living room, kitchen, bathroom and trash disposal
Assist the consumer in the purchasing of groceries and local errands at the most suitable, economic and efficient location.
Assist the consumer in the washing of personal laundry at the closest and most suitable location or complete the consumer’s laundry if appropriate.
Document any changes in the consumer’s condition and/or environment needs hourly.
Report any concerns to your supervisor.
Become completely familiar with the consumer’s record.
Perform other duties as assigned.
Read, understand and comply with all agency policies and procedures.
