Position Summary
CNS — the second-largest nonprofit Home Health and Hospice company in Utah — is seeking a compassionate, experienced Nurse Practitioner to provide primary care services to home health patients across CNS's home health service areas in Utah. You'll bring meaningful 1:1 patient care, significant clinical autonomy in a home-based practice setting, and the opportunity to manage a diverse caseload of adult and geriatric patients, with some pediatric patients depending on your population focus and certification. *Active Utah APRN licensure is required for this position.

Compensation & Benefits
- Salary: Starting at $110,00 more based on experience and background. .
- Mileage Reimbursement: Mileage reimbursement for qualified travel.
- Classification: Full-Time, Exempt, Salaried Employee.
- Schedule: Monday - Friday, 8am to 5pm
- On-Call: This position requires on-call availability as needed, which may include monthly rotation, holiday rotation, and coverage for patient needs.

Key Responsibilities
- Provide excellent care within the scope of your practice and licensure as defined under the Utah Nurse Practice Act.
- Diagnose and manage acute and chronic medical conditions, developing and adjusting individualized plans of care.
- Prescribe medications, including controlled substances, in accordance with Utah APRN licensure, DEA registration, and Utah Controlled Substance licensure.
- Order, perform, and interpret diagnostic tests and studies, adjusting treatment plans based on results.
- Collaborate with the interdisciplinary care team, including RNs, social workers, chaplains, and home health aides, to coordinate patient-centered care.
- Facilitate goals-of-care conversations with patients and families and support advance care planning.
- Complete timely, accurate clinical documentation in the electronic medical record in accordance with Medicare, Medicaid, and OASIS documentation requirements.
- Provide patient and family education regarding diagnoses, treatment plans, medications, and disease management.
- Participate in the on-call rotation, providing after-hours clinical support as needed.
Qualifications
- Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program; Family Nurse Practitioner (FNP) certification preferred for cross-lifespan care, though Adult-Gerontology (AGNP) may be considered depending on patient population.
- Active, unrestricted Advanced Practice Registered Nurse (APRN) license in good standing with the Utah Division of Occupational and Professional Licensing (DOPL).
- Current national certification as a Nurse Practitioner through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners Certification Board (AANPCB).
- Active DEA registration and Utah Controlled Substance license.
- Current CPR/BLS certification (or obtained within 30 days of hire).
- 1–2 years of clinical experience as a Nurse Practitioner; experience in home health, palliative care, or primary care strongly preferred.
- Prior Registered Nurse experience in home health or a related setting is a plus.
- Working knowledge of Medicare and Medicaid Conditions of Participation, OASIS eligibility and certification requirements.
- Strong communicator with the ability to build rapport with patients, families, and the interdisciplinary care team.
- Ability to work independently and exercise sound clinical judgment with minimal supervision in diverse home settings.
- Valid driver's license, current auto insurance, and reliable transportation.
- Ability to pass a criminal background check and drug screening.
Physical Requirements
- Ability to lift, transfer, and reposition patients as needed to perform physical assessments (up to 50 lbs frequently, more with assistance).
- Ability to drive throughout the CNS office you are assigned to and the surrounding areas.
- Ability to sit, stand, walk, bend, kneel, and squat for extended periods while conducting patient exams.
- Ability to navigate stairs and various home environments.
- Physical stamina to provide hands-on patient care throughout the day.
