Family Services of Westchester, Inc. (FSW) provides mental health and social services that strengthen and support families, children and individuals at every stage of their lives. Serving 30,000 people a year, including 20,000 children, with nearly 50 innovative programs, including counseling, education and support, FSW is a pillar of health in the Westchester community, here to create hope, restore dignity and connect families with the resources to get back on track.

JOB SUMMARY

Family Services of Westchester is seeking a Part-Time Home and Community Based Clinician to join our Child and Family Treatment and Support Services program in White Plains, NY.

This role will focus on providing supervision, support, and social skills building for children between the ages of 5-17. Other Licensed Practitioner (OLP) along with CFTSS services are goal-oriented supports and solution-focused interventions that are available:

To all Medicaid eligible children under age 21 who meet medical necessity criteria
Allow interventions to be delivered in person either at client’s home or other natural community-based settings where children and their families live as well as family/caregiver, or community collateral supports (as available/needed).
Provide lower intensity services to prevent the need for more restrictive settings and higher intensity services

RESPONSIBILITIES

Identify strengths and abilities through individual and family therapy.
Complete Licensed Evaluation, (Assessment), identifying a child/youth’s current mental, physical, and behavioral condition and history, which is the basis for establishing a diagnosis and treatment plan.
Conduct Treatment Plans that describe the child/youth’s condition and services needed for the current episode of care, detail of scope of practices to be provided, expected outcome, and expected frequency/duration of the treatment for each Provider.
Psychotherapy to work towards reducing symptoms or functional limitations associated with a child/youth’s diagnosed behavioral health disorder.
Provide Crisis Intervention Activities to provide the necessary interventions in crisis circumstances: crisis triage (by telephone), crisis off-site (in-person), and crisis complex care (follow-up)
Engage in wrap-around & collateral contacts with community partners for information gathering, to address any concerns to ensure effective communication to support the youth, family, and their support system.
Complete discharge plans as needed.
Maintain all documentations in Electronic Health Records as required.
Complete all agency required trainings at time of hire and annually.
Check and respond to agency emails/phone communications on a routine basis.
Meet with supervisor at least monthly and attend monthly staff meetings as scheduled.
Submit your schedule weekly to the program supervisor.
Maintain FSW agency vehicle’s cleanliness and document vehicle usage and maintenance needs.
This position may require the use of a personal vehicle with reimbursement for mileage.

QUALIFICATIONS

EDUCATION

LMSW, LCSW, LMHC, LCAT, LMFT, Licensed Psychoanalyst, Licensed Psychologist
Valid NYS Limited Permit acceptable

SKILLS

Ability to connect with young people/families, recognizing and building on their individual strengths.
Ability to set boundaries and limits, to work independently and collaboratively with others, and to communicate information clearly in a strength-based, culturally sensitive way, both orally and in writing.

EXPERIENCE

Experience providing clinical work with youth and their families
Training to work with trauma-exposed youth/complex trauma needs
Experience with the children’s mental health population is preferred.

ADDITIONAL REQUIREMENTS

Have own car and valid NYS driver’s license
Bilingual (English Spanish) a plus
Initial orientation and supervision, as well as training sessions, are required.
