Position Title: 099 Support Services Facilitator
Organization: AFA HUB
Location: Remote (in California)
Employment Type: Part-Time
Reports To: Director

AFA HUB provides innovative educational, vocational, and employment-focused services for individuals with developmental disabilities. We are committed to person-centered planning, self-determination, and helping participants achieve meaningful goals related to education, employment, independence, and community inclusion.

Position Summary
AFA HUB is seeking a compassionate, organized, and person-centered 099 Support Services Facilitator to support participants enrolled in California's Self-Determination Program (SDP). The facilitator will assist participants and families in implementing their Individual Program Plans (IPP), navigating services and supports, coordinating providers, and helping participants maximize their SDP budgets to achieve their goals.
The ideal candidate is knowledgeable about person-centered practices, developmental disability services, and the principles of self-determination.
Essential Duties and Responsibilities

Facilitate person-centered planning discussions with participants and families.
Assist participants in identifying goals related to education, employment, independent living, community participation, and personal development.
Help participants understand and utilize their SDP budgets effectively.
Coordinate and communicate with regional centers, fiscal management services (FMS), service providers, and family members.
Assist with identifying and securing appropriate services, supports, and community resources.
Monitor implementation of approved spending plans and support strategies.
Maintain accurate participant records and documentation.
Participate in meetings, including IPP meetings, planning meetings, and team consultations.
Provide ongoing support, advocacy, and problem-solving assistance.
Track participant progress and prepare reports as required.
Support participants in exercising informed choice and control over their services.

Qualifications
Required

Bachelor's degree in Human Services, Psychology, Social Work, Rehabilitation, Special Education, or related field; equivalent experience may be considered.
Minimum one year of experience working with individuals with developmental disabilities or related populations.
Strong organizational and communication skills.
Ability to manage multiple participants and priorities.
Proficiency with Microsoft Office, Google Workspace, and virtual meeting platforms.
Ability to work independently in a remote environment.

Preferred

Experience with California Regional Center services.
Knowledge of the Self-Determination Program (SDP).
Experience with person-centered planning.
ACRE, rehabilitation, employment services, or case management experience.
Bilingual skills highly desirable.
