Full Job Description

TLC Nurse Solutions is seeking experienced and compassionate Registered Nurses (RNs) to provide one-on-one private duty nursing care for a medically complex lung transplant patient transitioning home.

This opportunity is anticipated to begin in late September or early October. Nurses should be available to work 12-hour day shifts, 7:00 AM–7:00 PM. This case offers the potential for long-term continuity of care.

TLC Nurse Solutions also coordinates home infusion and IVIG nursing opportunities throughout California. Nurses with IVIG, infusion therapy, and specialty pharmacy experience are encouraged to apply, as additional opportunities may be available.

Responsibilities

Provide skilled one-on-one nursing care in the patient's home.
Perform comprehensive nursing assessments and ongoing monitoring.
Administer medications and treatments as ordered by the physician.
Monitor respiratory status and recognize changes requiring intervention.
Monitor for signs and symptoms of infection, rejection, or other post-transplant complications.
Collaborate with physicians, transplant coordinators, specialty pharmacies, and other members of the healthcare team to promote continuity of care.
Provide medication management and patient/family education.
Assist with meal planning, simple meal preparation, and nutritional support based on the patient’s dietary needs, preferences, and established care plan.
Assist with activities of daily living (ADLs) as needed, including bathing, grooming, dressing, toileting, personal hygiene, mobility, transfers, ambulation, and other supportive care while promoting the patient's independence, dignity, and safety.
Accurately document all assessments, interventions, and patient responses.
Maintain a safe, clean, and professional care environment.

Qualifications

Current California Registered Nurse (RN) license in good standing.
Minimum of two years of recent RN experience.
ICU, PCU, Step-Down, Pulmonary, Cardiac, Transplant, Acute Care, or high-acuity home care experience preferred.
Experience caring for medically complex patients in the home or acute care setting preferred.
Experience with central lines (PICC/CVC), tracheostomies, ventilators, transplant patients, or other high-acuity clinical care is a plus.
IVIG and/or home infusion experience is a plus and may provide opportunities for additional assignments through TLC Nurse Solutions.
Excellent assessment, communication, and critical-thinking skills.
Current BLS certification required; ACLS preferred.
Professional liability (malpractice) insurance.
Reliable transportation.

Independent Contractor Opportunity

This is a 1099 independent contractor opportunity coordinated through TLC Nurse Solutions. We are seeking experienced, dependable Registered Nurses who value professional autonomy while providing exceptional, patient-centered care.

Pay: $70.00 - $85.00 per hour
