Full Job Description

Job Summary

We are seeking a dedicated and experienced Senior Center Social Worker to join our dynamic team. In this vital role, you will provide comprehensive social work services to diverse populations within the community, focusing on promoting mental health, enhancing patient well-being, and supporting individuals with complex needs. Your expertise will help foster a compassionate environment where clients receive personalized care, advocacy, and evidence-based interventions. The Senior Center Social Worker will play a key role in case management, clinical assessments, and multidisciplinary collaboration to ensure holistic support for all individuals served.

Responsibilities

Conduct thorough intake assessments, clinical evaluations, and diagnostic evaluations using medical terminology and adhering to clinical documentation standards.
Develop and implement individualized treatment plans incorporating evidence-based practices such as cognitive-behavioral therapy, psychodynamic therapy, behavioral therapy, and applied behavior analysis.
Provide individual and group counseling sessions tailored to clients’ needs, including mental health counseling, addiction counseling, crisis intervention, and specialized care for conditions such as PTSD, autism spectrum disorders, Alzheimer’s disease, and disabilities.
Coordinate care through case management activities—facilitating referrals to community resources, home care services, special education programs, and medical providers—while ensuring compliance with HIPAA and medical confidentiality policies.
Maintain accurate medical records and documentation within EMR/EHR systems; ensure all clinical records meet standards for medical social work and legal requirements such as CPT coding, ICD-9/10 coding.
Engage in data collection and research initiatives to support program development and quality improvement efforts in behavioral health and mental health services.
Collaborate with multidisciplinary teams—including physicians, psychologists, educators, and community organizations—to optimize patient care plans and support holistic well-being.

Qualifications

Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution; licensure or certification as required by state law.
Proven experience working with children, adolescents, adults with developmental disabilities or disabilities in general, geriatrics including Alzheimer’s care, or psychiatric patient populations.
Strong knowledge of psychotherapeutic modalities such as cognitive-behavioral therapy (CBT), psychodynamic therapy, crisis management techniques, and evidence-based practices in behavioral health.
Familiarity with EMR/EHR systems; experience with ICD coding (ICD-9/10), CPT coding; understanding of medical terminology and clinical documentation standards.
Experience working in hospital settings or home & community care environments preferred; knowledge of HIPAA regulations is essential.
Excellent communication skills with the ability to conduct intake interviews, patient assessments, family counseling sessions—including child & family counseling—and group therapy sessions effectively.
Ability to manage crisis situations efficiently while demonstrating cultural competence and sensitivity to diverse populations.
