Job Overview
We are seeking a compassionate and energetic Case Manager to join our dynamic team. In this vital role, you will provide comprehensive support to children, youth, and families facing complex challenges. Your expertise in crisis intervention, behavioral health, and social work will help clients navigate their paths toward stability and growth. This position offers an opportunity to make a meaningful difference in the lives of those who need it most.

Required Qualifications and Skills:

Strong clinical skills, including assessment, interviewing and intervention.
Effective communication skills, in both verbal and written communication.
Strong organizational skills, including the ability to manage multiple tasks.
Cultural humility and ability to work with diverse populations.
Ability to maintain and ensure confidentiality.
Strong advocating skills including the ability to appropriately intervene on behalf of a client.
Possess a valid and clean driver’s license & record, willingness to transport clients in Company vehicles.
Knowledge of human behavior, child and adolescent development, normal and abnormal psychology.
Knowledge of child welfare systems, effective intervention strategies for victims of trauma, child abuse and neglect, and related emotional issues.
Basic computer skills including use of Microsoft Office Word, Outlook and Excel
Bi-lingual skills are a plus.

Essential Responsibilities:

Complete intake screening assessments and interviews, gathering necessary information from potential clients to determine eligibility for services provided by the agency.
Develop individualized service plans and link residents to appropriate community resources and social service agencies.
Review and update service plans regularly to reflect residents' evolving needs and goals and assist residents with obtaining benefits and eligibility for community services.
Provide treatment services as needed, including ongoing assessments, group facilitation, and crisis management.
Monitor resident progress through regular interventions, assess strengths and needs, and adjust support accordingly.
Research and maintain knowledge of community resources, benefits, and eligibility criteria.
Serve as a liaison with family members, referring agencies, and other involved stakeholders.
Coordinate and participate in treatment team and program meetings to evaluate resident progress and plan services.
Communicate regularly with team members regarding residents’ progress, concerns, and service coordination.
Develop and implement discharge plans to support resident transition and long-term stability.
Attend and participate in required meetings, workshops, conferences, and court hearings as necessary.
Maintain complete, accurate, and timely case notes, service plans, and reports using the AWARDS software system.
Drive residents, as needed, to appointments related to mental health, education, or other service areas.
Participate in performance & quality improvement processes by collecting and analyzing outcome data and recommending program changes including sharing relevant professional development insights with colleagues and contributing to staff learning.

Pay: $55,000.00 - $60,000.00 per hour
