Full Job Description

BASIC FUNCTION:The Facility Case Manager/Vocational Evaluator provides comprehensive vocational evaluation and case management services to individuals with disabilities participating in DVRS Extended Employment programs. Through formal and informal assessments, situational evaluations, community-based work assessments, and direct observation, the FCM/Vocational Evaluator identifies each individual's vocational strengths, interests, aptitudes, transferable skills, support needs, and employment barriers.

The Facility Case Manager/Vocational Evaluator coordinates and conducts Community-Based Work Evaluations and other vocational exploration activities. Working closely with consumers, referral sources, employers, and interdisciplinary teams, the FCM develops individualized vocational recommendations and goals based on person centered planning.

ESSENTIAL FUNCTIONS:

1. Coordinate intake for Evaluation, both traditional and CBWE referrals and DVR Extended referrals.

2. Perform prevocational evaluation on referrals admitted to evaluation, prepare written evaluation reports with recommendations for vocational goals and program services.

3. Complete invoice billing for services.

4. Provide vocational case management to consumers. Aid in the development of consumers' vocational knowledge, skills, and goal planning through case management services.

5. Observe and report on quality and quantity of consumers’ performance, general progress, goal attainment, vocational and/or personal needs. Relay information to liaison agencies, facility staff, family, consumer, etc., as required by designated program standards. Record observations and complete progress reports.

6. Record documentation pertinent to case management, updating as required. Maintain facility case record standards as required by CARF, Abilities' policies, Quality Assurance, CCW requirements and interrelated agency services. Enter, maintain, and update records in the electronic case management system..

7. Schedule and attend staff meetings involving consumer progress, Evaluation program, Individual Support Plans, Admission reviews, Program Evaluation, Quality Assurance, CARF standards, and others as deemed necessary. Complete documentation as applicable.

8. Gather monthly and annual statistical information. Record Daily Attendance Records for DVR Extended Employment, Evaluation, and other services. File consumer production records.

9. Manage consumers’ caseload through advocacy, counseling, service accessing and coordination, crisis intervention, program change facilitation, information and referral, travel arrangements, progress review and coordination of social services.

10. Perform other functions as they relate to the department and as assigned by direct supervisor.

11. Maintain CARF standards related to evaluation and organization employment.

12. Maintain professional confidentiality of case records, control case record access, and control release of information.

13. Distribute consumer payroll, notices and general information or delegate this responsibility.

14. Utilize computer network system for record keeping, compiling information, word processing, updating, etc.

15. Attend trainings that are required for maintaining certifications (Employment Specialist, Crisis Management, First Aid, etc.) and training to advance professional competency.

16. Will complete all mandatory trainings and professional development training hours per the direction of administration.

17. Collaborate with other case managers in case management department.

18. Adhere to all policies and procedures relate to the Electronic Health Records System.

19. Comply with the agency’s Ethical Conduct Policy.

20. Ensure the work environment is safe, healthy and positive for consumers served.

QUALIFICATIONS:

SKILLS:

5. Advocate for consumers with disabilities and facilitate program enhancements to benefit the consumers

8. Computer Skills

EDUCATION/LICENSES:

1. Bachelor’s degree in vocational rehabilitation, counseling, social work, or related field with 2+ years of experience in a related field which includes one year as a job coach.

2. Valid driver’s license and driving record in good standing.

EXPOSURE CATEGORY:

Category 2-Some exposure to Blood Borne Pathogens is likely.

WORK ENVIRONMENT:

This position operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, and printers/scanners.

PHYSICAL DEMANDS:

While performing the duties of this position, the employee is regularly required to speak and/or hear. The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms. The position may require lifting and long periods of standing.

TRAVEL:

Travel to Abilities locations may be required. Travel may also be required for certain training events as mandated by the state. You may be eligible for mileage reimbursement.
