Full Job Description

Job Summary

We are seeking a passionate and experienced PRN Hospice Field Registered Nurse to join our team. Aspen Grove Hospice is a mission-driven hospice dedicated to providing the highest quality of compassionate care to patients and their families during the end-of-life journey. We are building a team that is 100% focused on collaboration, mutual support, and a patient-first approach.

We employ the following systems to assist in our patient care: Relias learning management & CE platform, MatrixCare EMR, and CareXM triage and on-call system.

Duties

Provide holistic nursing care to in-home hospice patients, including symptom management, medication administration, and comfort measures.
Conduct thorough patient assessments to evaluate physical, emotional, and psychosocial needs.
Collaborate closely with physicians, social workers, chaplains, and other healthcare professionals in the IDT to develop and implement individualized care plans.
Manage care documentation and charting accurately and timely using Electronic Medical Record (EMR) systems while adhering to HIPAA regulations.
Educate patients and families about disease progression, medication use, and end-of-life choices with clarity and compassion.
Respond promptly to urgent patient needs through intake assessments and clinical case management.
Possibly provide 1-5 days of On-Call per month if desired (for additional compensation).

Requirements

Valid RN license with current state certification.
Minimum of 2 years hospice experience within the past 5 years is required.
Proven proficiency in using EMRs and documentation practices compliant with HIPAA; experience with MatrixCare is a bonus.
Excellent patient service skills with the ability to communicate effectively with diverse populations and their families.
Ability to manage multiple priorities in high-pressure situations while maintaining professionalism and compassion.
Valid Driver's License and auto insurance.
Must pass background checks.
