Full Job Description

Position Overview:

As a PRIDE Day Program Activity Coordinator, you will play a crucial role in providing support and guidance to individuals with intellectual and developmental disabilities. Your responsibilities encompass fostering social skills, daily living skills, and facilitating community integration. Your primary focus is to enhance participants' independence, safety, and overall well-being. Job Responsibilities:

1. Ensures Client Safety:

Adhere to program policies and procedures to maintain a safe environment for all

program participants.

Remain vigilant to identify and mitigate potential risks to client safety.

2. Skill Support and Activity Facilitation:

Encourage participants in the development of skills of daily living, social skills, and

community integration through activity facilitation.

3. Progress Notes:

Accurately and timely document and maintain comprehensive progress notes for

assigned participants using our Awards Foothold software by tracking individual

achievements, challenges, and areas for improvement along with the staff support

that are provided.

4. Activity Planning and Development:

Collaborate with colleagues to create engaging and meaningful activities that align

with the programs' goals and clients' individualized plans.

Adapt activities to cater to the diverse needs and preferences of the participants.

5. Transportation Duties:

Operate program owned vehicles to transport clients to and from community activities

and outings.

6. Mobility Assistance:

Provide physical support and assistance to clients with mobility challenges, as needed.

7. Self-Care Support:

As needed support participants with self-care including personal hygiene, dressing,

mealtime needs, etc.

8. Complete DDD and Pride Required Annual Trainings

A valid NJ Driver’s License is required; computer literate preferred.

Hours 8:30am-3:30pm
