Position Title: Senior Services Case Manager (Bilingual English/Spanish Speaker)

Position Summary
Responsible for implementing the Case Management Program services for elderly and disabled adults based on an assessment of their needs and consistent with all applicable policies and procedures.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Plans, administers, and manages the Case Management Program for senior citizen and disabled adults; processes intakes, makes referrals for other services, provides information, completes assessments to determine client needs, documents case records, handles/resolves emergency situations, follow-ups as necessary, checks progress toward identified client needs and goals, completes other tasks as necessary in order to ensure effective quality social service delivery.
- Identifies needs of elderly clients with respect to practical problems such as social security, housing, budgeting, home delivered meals, planning, and medical needs. Provides information (i.e. bus schedules) or direct assistance (i.e. transportation) to help client meet identified needs. Helps clients develop schedule for meeting identified needs (i.e. completing required paperwork, visiting appropriate agencies), trouble shoot on behalf of client if any special problems/conflicts arise, maintain current client status report and case logs in client files.
- Monitors and assesses client’s progress through frequent contact with client and/or community service providers.
- Develops and maintains regular contact with community services/agencies relevant to the client’s needs, refers, contacts, or acts on behalf of client with appropriate agency such as social security, Department of Public Social Services, legal clinics, district attorney, landlords, etc.
- Implements and manages all associated record-keeping systems including a system for performing individual assessments and developing individualized service plans including goals, time frames, selection of appropriate services, and provision for periodic re-assessment; ensuring accuracy and completeness.
- Maintains work hours within the budget and completes forms documenting the same. Completes assignments, case management performance goals and documentation according to contractual funding requirements.
- Participate in staff meetings and in-service training to develop and improve case management skills and to coordinate activities and services.
- Maintains confidentiality in all matters of the Agency/Program personnel, operations and clients; and maintains professional relationships with clients at all times.
- Maintains working knowledge of community services and resources available for clients.
- Performing other duties as assigned.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Bachelor’s degree required in social work or human services a minimum of one year of full-time paid or volunteer experience in case management or related field.
- Bilingual skills in English and Spanish; able to speak, translate, and write.
- Proficient in English grammar and spelling with excellent oral and written communications.
- The ability to type [50] WPM accurately; proficient in use of 10-key and other office equipment; intermediate computer skills including experience with Microsoft Office products, including but not limited to Word, Excel, and Outlook; ability to learn and use the PC hardware and software.
- Ability to communicate effectively with clients, family members, service providers, and co-workers with respect and dignity.
- Knowledge of human behavior and the aging process.
- Knowledge of community resources and available funding sources.
- Knowledge of the quality of services recommended.
- Knowledge of social and health intervention techniques.
- Effective case management and social services delivery abilities.
- Working knowledge of information and referral programs and the ability to develop and expand.
- Knowledge of and sensitivity to issues affecting aging adults, their families, and the community.
- Ability to work effectively with volunteers, small groups, advisory boards, and the community.
- Excellent verbal and written communication skills.
- Working knowledge of community based services and the techniques involved in utilizing these services working with clients in a community based social service modality.
- Ability to comply with client confidentiality guidelines and policies.
- Willingness to attend training seminars and meetings outside normal working hours.
- Must have a proactive and positive attitude toward clients, colleagues, board members, and community partners.
- Ability to relate and interact with aging adults in a positive and supportive manner, while offering them the opportunity to progress in the case management process as his/her/their own determined degree of involvement.
- Ability to positively participate as a part of a work team.
- Ability to support the mission and philosophy of the YWCA which includes acceptance of multicultural diversity.
- Current California driver’s license and a driving record acceptable to the Agency’s insurance carrier.
