Hy-Vee Health Exemplar Care exists to Solve Health Care Challenges. We are scaling across the Midwest with one goal: build the most trusted, member-first direct primary care model in the Region.

Overview

We are seeking a Member Engagement Lead to partner with employer groups and increase awareness and utilization of services available through Hy-Vee Health Exemplar Care clinics. Working closely with the Director of Member Services, this role supports onboarding, member outreach, education, and engagement efforts to help employees and their family access and make the most of their Hy-Vee Health Exemplar Care membership.

The ideal candidate is a proactive, relationship-driven professional who enjoys connecting with people, presenting information, and building strong employer partnerships. This role serves as a key resource for assigned groups, leading engagement initiatives, on-site activities, and ongoing communication that encourages members to utilize the healthcare services their employer provides.

This is a full-time, hybrid position based in Des Moines, IA, with occasional regional travel for employer meetings, onsite engagement activities, and events. Join us in creating meaningful healthcare experiences and improving access to care in the communities we serve.

Primary Responsibilities

Serve as the primary relationship manager for assigned employer groups and member accounts
Lead onboarding for new members, ensuring a smooth introduction to Hy-Vee Health Exemplar Care
Develop and execute engagement strategies to increase awareness and utilization of available services
Conduct regular virtual and in-person engagement meetings with employer groups to strengthen relationships, gather feedback, and identify opportunities to improve member participation and experience
Coordinate employer group billing, eligibility, and related member account activities
Analyze engagement and utilization data to identify trends and opportunities to improve participation and member experience
Support open enrollment periods, employer meetings, events, and on-site engagement opportunities

Preferred Qualifications

Bachelor’s degree or equivalent experience
3–5+ years in account management, customer success, or member/patient engagement
Strong communication and relationship-building skills
Ability to manage multiple accounts and priorities effectively
Experience in healthcare, benefits, or membership-based services preferred

Work Schedule

· 40 hours per week

Work Location and Travel Requirements

· Ability to travel regionally for employer meetings and onsite engagements as needed

Salary

· $55,000 - $60,000 (Negotiable)
