Job Overview
Illuminate Hospice is the first and only provider specifically designed for skilled nursing patients and their care teams. We currently offer our services in over 35 nursing homes throughout the Chicagoland area. Our mission is to provide compassionate care, ensuring dignity, respect, and quality of life for our patients while supporting their families and care providers. Duties:

Be available for urgent patient needs, quickly responding to calls and making in-person visits as required.
Conduct initial and on-going comprehensive psycho-social assessments of patients' physical, emotional, family and spiritual needs.
Provide direct social work care following the established care plan
Develop and implement care plans to meet the emotional, social, and spiritual needs of patients
Facilitate communication between patients, families, and other healthcare professionals
Provide crisis intervention and support to patients and families
Educate patients and families on end-of-life options and resources
Maintain accurate and up-to-date patient records and documentation
Assists patient/family in preparation for impending death, including such items as wills and funeral arrangements.
Counsels the patient/family as they work through their acceptance of the death and dying process and the grief process.
Serves as member of patient care team, interpreting/identifying psychosocial needs of the patient/family.
Assists with volunteer training on relevant subjects.
Assesses bereavement needs of survivors and communicates these to bereavement coordinator.
Facilitates Support Groups
Provides Education and training to community groups/nursing facilities, as directed
Offer emotional support and guidance to patients and their families
Coordinate with other healthcare professionals to ensure comprehensive care
Maintain accurate and timely patient records and documentation in the Electronic Health Record (EHR)
Participate in interdisciplinary team meetings to plan and review patient care
Assess patient needs and adjust the care plan as necessary
Assist with pain management techniques/coping skills and interventions
Ensure compliance with all hospice and healthcare regulations and protocols
Deliver empathetic nursing support to patients in nursing homes, prioritizing comfort and dignity.

Requirements:

Master’s degree in social work -MSW (Required)
Reliable transportation & valid driver’s license
Knowledge of infection control practices and emergency medicine protocols.
Excellent communication skills, both verbal and written.
Compassionate demeanor with a commitment to providing empathetic care.
Ability to handle emotionally charged situations with empathy
Willingness to travel within the assigned area for patient visits
