Overview

Join our compassionate team as a Home Health Aide & Caregiver, where your dedication and skills will make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will provide essential support to patients in their homes, promoting independence, comfort, and well-being. Whether assisting with daily activities or offering specialized care, your efforts will empower clients to live safely and happily in familiar surroundings. Duties

Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
Monitor and observe patient health status by recording vital signs, noting changes in condition, and reporting findings accurately in Electronic Health Record (EHR) or Medical Record systems.
Administer medications and perform catheter care following medical terminology guidelines and home healthcare regulations.
Support clients with meal preparation, light cleaning, laundry, and other household tasks to maintain a safe and comfortable environment.
Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments through memory care techniques and patience.
Document all patient interactions meticulously while maintaining strict compliance with HIPAA standards to ensure confidentiality.
Assist with patient transfers using proper patient transfer methods or equipment such as Hoyer lifts or gait belts while ensuring safety protocols are followed.

Skills

Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or end-of-life care.
Strong knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (CPR/First Aid).
Familiarity with EMR/EHR systems for accurate medical documentation and home health agency procedures.
Excellent communication skills to effectively interact with clients, families, and healthcare teams while maintaining professionalism and compassion.
Ability to perform heavy lifting when necessary and adhere to safety standards for patient transfer methods.
Experience working within home health care regulations and understanding of HIPAA compliance to protect patient confidentiality.
Capable of managing multiple responsibilities including cooking, cleaning, laundry, and providing emotional support tailored to each client’s needs.
