Job Title: Service Coordinator

FLSA Status: Hourly, Non-Exempt

Classification: Full- Time

Reports To: Service Coordinator Manager

Revision Date: 03-20-2026

Job Purpose

The Service Coordinator (SC) serves as a primary support and resource for residents, facilitating access to social, supportive, and medical services to help them remain independent and improve their quality of life. The SC empowers residents to age in place, build informal support networks, and maintain self-reliance while working in conjunction with management staff. Services may include activities of daily living (ADLs), education on available resources, and connections with community service providers.

Duties & Responsibilities

Provide limited case management and referral services for all residents needing assistance.

Educate residents, families, and staff on available community resources, service application procedures, and residents’ rights.

Assist residents in building informal support networks with other residents, family, and friends.

Act as a liaison for residents between community agencies, service providers, and volunteers.

Assist residents with the delivery of health and home care services, to ensure they are timely, appropriate, and satisfactory.

Encourage residents to proactively address social, psychological, and physical needs while avoiding the creation of unhealthy dependence.

Facilitate educational programs and wellness activities that promote independence and self-reliance.

Maintain confidentiality and comply with all relevant HUD, state, and federal regulations.

Maintain secure resident files documenting intake information, service plans, progress notes, and follow-up actions.

Prepare reports for site management, quality assurance, and government agencies as required when applicable.

Develop and maintain a directory of community services for use by residents, their family and staff.

Stay current on statutes, policies, and required AASC and HUD training/certifications.

Advocate on behalf of residents for adequate, timely, and cost-effective services.

Key Performance Measurement(s)

Community Outreach: Develops and maintains strong relationships with community partners to expand available resources and improve resident access to supportive services.

Compliance Reporting: Ensures all required reports are completed accurately and submitted on time in accordance with organizational, state, federal, and HUD regulations when applicable.

Educational Outreach: Designs and delivers educational programs and workshops that promote resident independence, health, and overall well-being.

Professional Communication: Demonstrates clear, timely, and professional communication with residents, staff, and external partners, fostering trust and collaboration.

Resident Support: Provides effective coordination of services that address resident needs while promoting self-sufficiency and enhancing quality of life.

Physical Activities & Requirements

Ability to sit, stand, and walk for extended periods (up to 100% of the work shift).

Ability to lift, push, or pull up to 50 lbs. as needed.

Average hearing and vision required for communication and inspection duties.

Must possess a valid driver’s license and maintain a clean driving record.

Qualifications

Bachelor’s degree in Social Work, Gerontology, Psychology, Public Health, or related field preferred but not required.

Minimum 2 years of experience working with the senior population or in service coordination.

Knowledge of community services, benefits and entitlement programs, elder services, disability services, and mental health issues.

Strong communication, writing, organizational, problem-solving, and advocacy skills.

Ability to maintain confidentiality and professional boundaries.

Awareness of legal liability issues related to service coordination.
