Position Summary

Avid Health at Home is seeking compassionate Registered Nurses (RNs) to provide skilled private duty nursing care to clients in their homes throughout the Wilmington area.

Working under the supervision of the Clinical Manager, you'll provide individualized nursing care while helping clients remain safe, comfortable, and independent at home.

Responsibilities

Evaluate and treat clients according to their individualized plan of care under the supervision of the Clinical Manager.
Provide skilled, compassionate one-on-one nursing care in the client's home.
Assist in maintaining continuity of care while supporting the physical, emotional, and psychosocial needs of clients and their families.
Build trusting relationships that promote client safety, comfort, and compliance with the plan of care.
Use sound nursing judgment to assess client needs and respond appropriately to changes in condition or emergencies.
Observe and report changes in the client's physical, emotional, or mental status to the Clinical Manager or Customer Care Coordinator.
Maintain accurate and timely clinical documentation, shift notes, and all required communications.
Ensure continuity of care through effective communication with clients, families, physicians, health plans, and the interdisciplinary care team.
Demonstrate proper use of medical equipment and adhere to infection prevention and safety guidelines.
Maintain the highest standards of customer service, professionalism, and confidentiality.
Participate in staff education, professional development, and community outreach activities.
Comply with HIPAA, Agency Policies and Procedures, and all applicable regulatory requirements.
Perform all other duties as assigned.

Qualifications - Education

Graduate of an accredited school of nursing.

Licensure / Certification

Current, unrestricted Registered Nurse (RN) license issued by the North Carolina Board of Nursing (NCBON).
Must remain in good standing and maintain all state and federal licensure requirements.

Experience

Minimum of one year of home care experience.
One additional year of nursing experience, preferably in a hospital setting, is required for conducting applicable training classes.

Travel

Travel is required between client homes.
Valid driver's license and satisfactory driving record are required.
