Full Job Description

Here at the Trauma-Informed Counseling Center, also known as TICC, we strongly believe in providing meaningful support, guidance, and empowerment to our clients. As TICC continues to grow we are now hiring Client Case Coordinators/Targeted Case Manager’s to help us keep growing and serving the needs of our clients. Responsibilities for Client Care Coordinator/Targeted Case Manager

· Coordinate and facilitate client care through assessment, evaluation, planning, and implementation

· Communicate client needs to a variety of care team members and follow up accordingly

· Manage discharge plans upon completion of services

· Work collaboratively with clients, families, physicians, and nurses to ensure high quality care

· Act as the clients advocate as it relates to insurance coverage and financial assistance

· Maintain the client’s comprehensive clinical record through detailed documentation

· Coordinate with third party payors on a regular basis

Qualifications for Case Manager

· Bachelor's degree in Art Sciences, Social Work, Psychology or other related field.

· Certified Case Manager preferred but not required, however, applicants MUST possess foundational case management training that is required by the state of Kentucky.

· 1-2 years of clinical experience, preferably in case management, or related experience.

· Ability to walk and stand 50-80% of the time.

· Proficient computer skills including experience with Microsoft Excel and Microsoft Word.

· Excellent written and verbal communication skills.

· Highly proficient clinical assessment and analytical skills.

Pay: $500.00 - $3,240.00 per month
