Note, this role will travel within Milwaukee North territory. This includes the following counties: Waukesha, Washington, Ozaukee, Milwaukee.

What You Will Do

Build and maintain therapeutic relationships with hospice patients and families, delivering compassionate, individualized, and holistic care across all settings.
Assess patient needs, pain, symptoms, and responses to treatment; continuously update care plans and interventions to optimize outcomes.
Collaborate with physicians, caregivers, and interdisciplinary teams to coordinate care, discharge planning, referrals, and support services.
Educate patients, families, volunteers, and caregivers on disease progression, treatment plans, hospice care, and symptom management.
Promote quality, safety, and regulatory compliance through evidence-based practice, process improvement initiatives, and effective resource utilization.
Apply sound clinical judgment, prioritize complex patient needs, maintain accurate documentation, and ensure continuity of care.
Demonstrate leadership and professional growth through teamwork, communication, ongoing education, certification maintenance, and adherence to accreditation, HIPAA, and organizational standards.

Schedule Details

Part time
This is every Saturday and Sunday 8a-8p working in the Milwaukee North territory. This includes the following counties: Waukesha, Washington, Ozaukee, Milwaukee.

What We Are Looking For

Registered Nurse – Current Wisconsin RN license
A valid driver’s license issued by the Division of Motor Vehicles.\
Current CPR
Graduate from an accredited school of nursing
Minimum 1 year of experience
Earn 10.0 continuing education credits annually
