Job Announcement: Case Manager for HS Program

for Workforce Development & Continuing Education

Status: Full Time, Union, In Person
Reports To: Program Director

Annual Salary: $48,000 - $50,000, commensurate with education and experience.

Schedule: Monday – Friday 9 AM – 6 PM, including Evenings and Weekends as needed.

The Mosholu Montefiore Community Center (MMCC) is a vital community resource that changes the course of lives of Bronx and Manhattan residents to a degree not otherwise possible. Each year, we serve more than 40,000 preschoolers, school-age children, teens, adults, and senior citizens through our support, enrichment, education, and recreation programs, expanding their opportunities to live full, productive lives.

Responsibilities:

Participant Case Management

· Conduct intake assessments of high school juniors and seniors in the Learn & Earn Program to identify participants’ employment, educational and support service needs.

· Develop individualized service plans outlining career and training goals.

· Maintain regular contact with participants through meetings, phone calls, emails and text messages.

· Monitor participant attendance, progress and engagement throughout program participation; and re-engage students at risk of dropping out of the program.

· Provide crisis intervention and referrals as needed.

Supportive Services Coordination

· Connect participants to school and community resources including housing, childcare, healthcare and mental health services.

· Advocate on behalf of participants to remove barriers to high school completion, employment and training completion and/or applying to higher education.

Employment and Career Support

· Coordinate with job developers to facilitate employment opportunities.

Documentation and Data Management

· Maintain accurate and timely case notes (due monthly at minimum) and participant records in both physical files and program databases to ensure compliance with funder requirement.

· Collect and verify participant documentation.

· Track participant outcomes, including enrollment, credential attainment, employment placements and retention; maintain client confidentiality.

Collaboration and Program Support

· Participate in staff meetings, case conferences and professional development activities.

· Collaborate with instructors, employers, school partner, community partners and other program staff to support participant success.

· Assist with program recruitment, orientations, workshops, community events and special events as needed.

· Participate in program documentation audits both internal and external.

· Perform additional office and program duties as requested by the department.

· Perform other related duties as assigned by the Program Director.

Qualifications

● Bachelor’s Degree and two years of relevant experience in case management, social services or workforce development.

● Experience working with diverse populations including youth, unemployed youth and adults and individuals facing employment barriers.

● Availability for some evenings and weekends as necessary.

● Strong knowledge of community resources.

● Demonstrated leadership, problem-solving, and time management skills.

Skills

● Excellent verbal and written communication skills.

● Strong organizational skills with exceptional attention to detail.

● Dependable, consistent, and results-oriented.

● Strong interpersonal skills with the ability to work independently and collaboratively.

● Ability to work effectively in a flexible, fast-paced environment.

● Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and Google Workspace.
