2 days at MV Norman P Murray home assessments

2 days Dana Point Senior Center home assessments

1 day Admin Office/Florence Sylvester Senior Center

Age Well Senior Services is a nonprofit, 501(C)(3) organization. Since1975, Age Well has been providing meals, critical services, resources, and programs to seniors living in South Orange County. We are committed to finding caring solutions to the evolving challenges that seniors face today.

Age Well’s Mission: To provide resources for older adults to promote their vitality and support their independence.

Position Summary

Age Well Senior Services is seeking a compassionate and organized Care Manager to support older adults throughout South Orange County. Our Care Management works collaboratively with seniors, caregivers, families, and community partners to assess needs, coordinate services, and help older adults remain joyful, active, and independent for as long as possible.

As part of the Care Concierge Program, this position provides individualized support, advocacy, resource navigation, and wellness-focused care planning for adults age 60 and older enrolled in Age Well nutrition programs. This position also confirms and clears participants for Friendly Visitor program.

Must have case management experience. Home Visit experience. Assessment experience. APS experience & /or knowledge preferred- understanding of mandated reporting. Comfortable with working with seniors and visiting them in their home environment.

- Essential Duties and Responsibilities

Care Management & Team Support

* Excellent communication skills.

* Must be knowledgeable with OC Resources for seniors.

* Must feel natural with all consulting with C2 (Meals on Wheels) & C1 (Lunch Program) Managers whenever needed; some calls have immediate needs to be assessed for APS, wellness checks. Provide education to C2 & C1 Managers.

* Monthly reports provided to administration and City Senior Center Staff.

* Call logging for type of information provided to seniors – statistical data/reporting by month by City.

* Goals, benchmarks: Increase referrals received by C2 managers.

* This includes inbound resource calls, referrals, in home assessment and evaluate for 3-month care program or Friendly Visitor Program.

* Develop individualized 3-month care plans based on participant goals and identified needs and evaluate physical, nutritional, emotional, and social support needs

* Conduct wellness checks, reassessments, and care coordination as needed

* Collaborate with Home delivered meals team to educate on referrals.

* Attend lunch programs as calendared to provide education to seniors regarding resources and navigating aging-related challenges

* Maintain accurate case notes and participant documentation in accordance with Age Well standards

* Work closely with Director of Grants and Foundations to ensure program funding.
