Full Job Description

About the Role: The Licensed Practical Nurse (LPN) or Registered Nurse (RN) on the Assertive Community Treatment (ACT) team works under the supervision of the ACT Team Leader, in collaboration with the psychiatric care provider, and (for RNs) the Lead RN. The LPN/RN provides psychiatric nursing services, including medication management, nursing assessments, treatment planning support, education, rehabilitation, and coordination of services and community resources to help individuals maintain or return to community living. The role includes making referrals to community physicians and supporting continuity of care through appropriate documentation and interdisciplinary collaboration. The LPN primarily focuses on direct nursing care and medication management within the scope of practice, while the RN primarily supports the nursing department through administrative coordination, documentation, and operational functions to ensure efficient daily operations and continuity of care. In the absence or unavailability of the Lead RN, the RN may also provide backup support for medication management and psychiatric nursing services, and may participate in community-based services as needed to support client care.

Responsibilities:
The LPN and RN on the ACT team participate in daily team meetings and provide nursing support, which may include rotating on-call coverage during evenings, weekends, and holidays. Both roles are responsible for medication administration and documentation, monitoring clients for medical and psychiatric concerns and side effects, coordinating care with providers, pharmacies, labs, hospitals, families, and community agencies, and ensuring medications are properly ordered, stored, and administered in accordance with regulations and professional standards. Both positions support health education, medication adherence, crisis response, advocacy, admissions/discharges, and coordination of services, while maintaining accurate documentation and HIPAA compliance.

The LPN primarily provides direct nursing care, client education, and support with medication adherence and care coordination. The RN additionally participates in treatment planning, provides nursing consultation and staff training, supports clinical oversight, and uses advanced clinical judgment in complex or high-risk situations.

Candidate Requirements: Knowledge, Skills, and abilities:

Both LPN and RN candidates must demonstrate knowledge of mental illness, including etiology, symptoms, diagnosis, and treatment modalities, as well as safe administration of psychotropic medications (indications, administration methods, expected effects, side effects, contraindications, and adverse drug interactions). Experience in mental health nursing is required. Candidates should also demonstrate knowledge of case management and social work practices, including the ability to access and utilize community resources, and must be willing to work with individuals with co-occurring serious mental illness and substance use disorders, as well as their families and support systems. Knowledge of applicable Medicaid guidelines and ethical/legal standards related to nursing practice is required. Strong written and verbal communication skills are essential, including the ability to clearly explain medical terminology to clients and families in an understandable way. Candidates must demonstrate initiative, resourcefulness, and the ability to work both independently and as part of a multidisciplinary team, along with basic proficiency in computer use and word processing. Both LPN and RN staff are required to maintain all continuing education units (CEUs) and annual training requirements for licensure.

Physical Requirements For LPN & RN:

Prolonged Period of sitting while driving up to 75% of the time

Must be able to be stationary standing up to 50% of the time.

Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.

Occasionally move items weighing up to 15 pounds across the office for various needs.

Consistently works in outdoor weather conditions or changing work environments.

The ability to observe details at close range (within a few feet of the observer).

The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.

Education and Experience for LPN & RN:

Graduate of an approved School of Nursing Program

A valid Registered Nursing License or Licensed Practical Nursing License from the Virginia Board of Health Professionals

At least one year of experience in the provision of mental health services to adults with severe mental illness

Pay: $27.00 - $30.00 per hour
