Full Job Description

About the Practice

Mohammad S. Ghalichi, M.D., A Professional Corporation is a private preventive cardiology, lipidology, and longevity medicine practice located in Los Angeles. The practice is designed around extended appointment times, comprehensive cardiovascular risk assessment, advanced lipid management, and a strong commitment to clinical excellence and patient continuity.

Position Summary

The Corporation is seeking a highly motivated, dependable, and detail-oriented Physician Assistant (PA-C) to join our clinical team. The Physician Assistant will exercise independent clinical judgment and discretion in the assessment, diagnosis, and treatment of patients within the scope authorized by a written Practice Agreement, in accordance with California Business and Professions Code Section 3502 et seq.

Responsibilities include comprehensive preventive cardiovascular care, independent management of medications and prior authorizations, independent ordering and review of laboratory and imaging studies, patient education, and ongoing clinical follow-up. This position offers the opportunity to develop deep clinical expertise in preventive cardiology, lipidology, and longevity medicine under the mentorship of a board-certified cardiologist.

The practice operates under a highly personalized care model that emphasizes comprehensive evaluations, extended appointment times, meticulous follow-up, and exceptional patient service. As a result, patient volume is intentionally lower than that of a traditional high-volume medical practice, and the daily pace may at times be less hectic than in many outpatient settings. However, patient needs, urgent clinical matters, and schedule changes can arise at any time.

Accordingly, the Physician Assistant is expected to maintain a consistent on-site presence during scheduled clinic hours unless otherwise approved by the Corporation. Even during periods when no patients are physically present in the office, the Physician Assistant will be expected to remain available to support patient care activities, review records, coordinate testing and treatment plans, complete clinical documentation and prior authorizations, respond to patient communications, and address other clinical or operational needs as they arise throughout the business day.

Essential Responsibilities

Independently assess, diagnose, and treat patients within the scope authorized by the written Practice Agreement, exercising independent clinical judgment and discretion.
Produce accurate and comprehensive clinical documentation, including history, physical examination, assessment, and plan, reflecting the Physician Assistant's independent clinical reasoning.
Manage prescription refill requests in accordance with the Practice Agreement and applicable California law.
Complete prior authorization requests for cardiovascular and metabolic medications, including PCSK9 inhibitors (e.g., Repatha), GLP-1 receptor agonists, and related specialty agents.
Independently place imaging orders, including cardiac CT, coronary artery calcium scoring, echocardiography, stress imaging, and vascular studies, based on clinical assessment, and coordinate scheduling and results review.
Independently order and review laboratory studies, including advanced lipid panels and screening assessments, based on clinical assessment, and communicate results to patients.
Deliver evidence-based patient education on cardiovascular risk reduction, medication adherence, and lifestyle modification.
Support clinic operations through inbox management, referral coordination, and care coordination with consulting specialists.

The successful candidate will demonstrate a strong work ethic and consistent reliability, sound independent clinical judgment, a genuine interest in cardiovascular prevention, lipidology, and longevity medicine, meticulous attention to detail in documentation, follow-up, and patient communication, excellent written and verbal communication skills, professional composure and discretion, and an eagerness to learn and grow.

Success in this role requires sound independent clinical judgment, professional accountability, and a commitment to evidence-based, patient-centered care.

The Corporation is committed to providing individualized training on practice-specific workflows, including preventive cardiology protocols, longevity medicine evaluations, advanced lipid management, diagnostic testing, and prior-authorization processes for specific medications and treatments.

Required Qualifications

Active and unrestricted California Physician Assistant license.
Current NCCPA certification (PA-C).
Current BLS and ACLS certifications.
Willingness to execute a written Practice Agreement, Confidentiality and Non-Disclosure Agreement, and Mutual Arbitration Agreement.
Eligibility to work in the United States.

Preferred Qualifications

Clinical experience in cardiology, internal medicine, endocrinology, lipidology, or primary care.
Familiarity with prior-authorization workflows.
Prior experience in a private, concierge, or direct-pay clinical setting.

Work Setting

Outpatient private practice, on-site in Los Angeles, CA 90048.
