Full Job Description

Cardinal Home Care is seeking a compassionate, organized, and motivated Licensed Practical Nurse (LPN) to join our growing team in Northern Virginia.

This is a unique opportunity to combine office-based clinical work with field visits, allowing you to play an important role in maintaining high-quality, client-centered care throughout our community.

What You’ll Do

As a Staff LPN, you’ll work closely with our Director of Nursing to ensure our clients receive exceptional non-medical home care while supporting and mentoring our caregiving team.

Your responsibilities will include:

Conduct client reassessments and recommend updates to care plans as clients’ needs change.
Perform caregiver supervisory visits and annual performance evaluations.
Participate in caregiver interviews, orientation, and hands-on skills assessments.
Review caregiver documentation and communicate concerns with the Director of Nursing.
Provide education and coaching to caregivers on topics such as dementia care, infection prevention, safety, and best practices.
Assist with care planning and maintaining compliance with Virginia home care regulations.
Help promote a culture of teamwork, accountability, and exceptional client service.

What We’re Looking For

Active Virginia Licensed Practical Nurse (LPN) license
Graduate of an accredited practical nursing program
Minimum one year of nursing experience
Current CPR certification
Home care, hospice, assisted living, or long-term care experience preferred
Excellent communication and organizational skills
Comfortable working independently and as part of a collaborative team
Reliable transportation for occasional client and caregiver visits throughout Northern Virginia with possibility of receiving a company car after the completion of the first 90 days
