Full Job Description

We are seeking an experienced and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality wound care services across multiple care settings. The provider will perform comprehensive wound assessments, develop individualized treatment plans, monitor healing progress, and collaborate with physicians and other members of the healthcare team to achieve optimal patient outcomes.

Perform comprehensive wound assessments and evaluate acute, chronic, and complex wounds.
Assess, measure, photograph, document, and track wound healing and progression.
Develop and implement individualized wound care treatment plans.
Perform and/or manage appropriate wound care procedures, including debridement, dressing selection, and topical treatments, within scope of practice.
Diagnose and manage relevant acute and chronic conditions and prescribe medications as clinically appropriate and permitted by scope of practice.
Order and interpret diagnostic testing, including laboratory studies, imaging, and vascular studies.
Monitor patient progress and modify treatment plans based on clinical findings and healing response.
Coordinate wound care with the management of underlying conditions that may affect healing, including diabetes, hypertension, vascular disease, and mobility limitations.
Educate patients, families, and caregivers regarding wound prevention, proper wound care, disease management, nutrition, and lifestyle modifications.
Collaborate with physicians, nurses, caregivers, facilities, and other members of the multidisciplinary healthcare team.
Maintain timely, accurate, and compliant documentation within the EMR.
Follow applicable clinical, regulatory, privacy, and documentation requirements.

Qualifications

Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license.
Minimum of 2 years of clinical experience, preferably in wound care, primary care, geriatrics, home health, or a related field.
Previous wound care experience strongly preferred.
Knowledge of wound assessment, staging, treatment, infection prevention, and healing principles.
Wound care certification such as WCC, CWS, CWCA, or equivalent is preferred but not required.
Strong clinical assessment, diagnostic, procedural, documentation, and communication skills.
Experience using EMR systems and mobile documentation technology.
Ability to work independently while collaborating effectively with a multidisciplinary team.
Ability and willingness to travel between patient care locations.
Bilingual English/Spanish preferred.

Compensation & Benefits : Part Time, 1099 Contract Position, Mileage Reimbursement
