Full Job Description

SUMMARY

Tatra Solutions is seeking a Nurse Practitioner (NP) to support our veteran community at the Armed Forces Retirement Home (AFRH) in Gulfport, MS. AFRH is a premier retirement community for our veterans through a residential quality care and supportive services environment.

The position is a full-time position with the hours of 7:00 am – 3:30 pm, M-F. The NP will be required to participate in an on-call rotation schedule with other staff.

DUTIES
The mission of the position is to provide professional medical and technical services to the Residents of AFRH and will work under the direct supervision of the Chief Medical Officer or Chief, Healthcare Services. Work is reviewed by supervisor, committees and audits in accordance with recognized Quality Assurance Guidelines. Duties include but not limited to:

Perform complete medical management in Resident all clinical care settings (Ambulatory, Long Term Care (LTC), Memory Support (MS) and Assisted Living (AL)). The incumbent assists in the following functions of development and execution of the medical management and professional activities in all clinical care settings. Performs complete physical examinations, collects, and records histories, orders appropriate laboratory studies, x-rays, electrocardiograms and other special procedures required to evaluate medical problems.
Makes on-going medical rounds with the medical team observing and assessing the Residents' physical conditions. Assesses and monitor acute and chronic medical conditions through follow-up visits and notifies physician when necessary, of changes in Resident's conditions, weight changes, medication errors, medications not available, abnormal lab and x-ray results and any other information as necessary.
Attends interdisciplinary care team conferences and has an active role in developing and implementing the plan of care. Allowed to serve as alternate to unit physician in his/her absence or as requested in special circumstances or specified cases.
Makes appropriate entries in the electronic medical record on Resident transactions, writing orders, recording progress notes, or writing/dictating discharge summaries.
Orders and interprets appropriate diagnostic studies. Reviews initial laboratory, x-ray and medical entries and interprets data, radiographic studies, electrocardiograms and other auxiliary diagnostic methods and then formulates with Resident input a plan for the solution of any medical problems. Initiates consultation requests with other health specialists, if necessary.
Prescribes, appropriate drug therapy and hydration therapy for acute, chronic and seriously ill medical conditions and makes the appropriate adjustments as needed. Reports, documents and completes necessary forms and medical records for adverse/allergic reactions to the Pharmacy and Therapeutics Committee. In collaboration with the primary physician, write orders on Residents to include medications for which authorized to prescribe. Ordering of Schedule III drugs, i.e., narcotics may be carried out.
Counsels and performs health care educational sessions with Residents on medical problems, use of drugs, expected effects of treatments, diet and other therapeutic and health matters.
Provides educational programs for Health Care Services staff as requested and/or appropriate.
The Nurse Practitioner performs a wide range of clinical procedures including but not limited to performs electrocardiograms, cleans and debrides wounds, sutures minor lacerations, applies dressings, packings, bandages, ear or eye irrigation, initiates cardiopulmonary resuscitation until a physician arrives on the scene or is relieved by certified officials.
Diagnoses and treats acute conditions promptly and appropriately (i.e., chest pain, exacerbation of COPD, uncontrolled hypertension/diabetes mellitus, acute intoxication, lacerations, etc.).
Transfers Residents to acute care facilities with completed Hand-Off communication process.
Accepts those Residents transfers back from acute care facilities and reconciles medications.
Participates in and supports performance improvement activities.
Maintains continuing medical education and certification/licensure as indicated.
Required to participate in Interdisciplinary Teams (IDT) meetings on regular and emergent basis.
Required to participate in Peer Review program.
Required to participate in Wellness Clinic appointments, hospital follow-up appointments, Sick Call and Coumadin Clinic
Take part in clinic Performance Improvement Quality Assurance Program as required to meet the standards of The Joint Commission and AFRH Standard Operating Procedures.
Serves on various committees as assigned. Serves as alternate to the physician chairperson on select committees. The NP shall be an active member and participant in healthcare committees which include, but are not limited to Needs Assessment Team, Performance Improvement, Infection Control, Mortality Review Committee, Pharmacy and Therapeutic and others designated by Chief, Medical Office.
The Nurse Practitioner may be assigned to a Disaster Team function and may be required to be on board in a working status before, during, and immediately following any natural disaster.

QUALIFICATIONS

U.S. Citizen
Must be able to pass a background investigation
Have a valid registered nurse (RN) degree from an accredited college or university certified by the National League of Nursing.
Licensed and credentialed as a Nurse Practitioner (NP) in Mississippi.
Certified as a Nurse Practitioner in Geriatrics or Family Health.
Must submit and pass AFRH privileging process.
Experience working with a geriatric population and the Wellness Model.
Effective oral and written communication skills in the English language.
Must possess a current Federal Drug Enforcement Agency (DEA) registration.
