Case Manager – Individual Supports / CBDS Programs

$21-$24/hour | Flexible Schedule | Community-Based Role | Sandwich, MA

Make a Difference with KDC

At Kennedy-Donovan Center (KDC), we're committed to empowering adults with developmental disabilities to live meaningful, independent lives within their communities. As a Case Manager supporting our Individual Supports (IS) and Community-Based Day Services (CBDS) programs, you'll play a vital role in helping individuals achieve personal goals, build relationships, access community resources, and strengthen independent living skills.

This flexible, hybrid position floats between our CBDS and Individual Supports programs, providing both direct support and case management services based on individual and program needs. You'll work closely with individuals, families, community providers, and interdisciplinary teams to promote choice, inclusion, and personal growth.

ASL fluency is strongly preferred.

Schedule

Flexible schedule; primarily weekday hours
Weekend availability (Saturday and/or Sunday) may be needed
CBDS Program Hours: Monday-Friday, 8:00 AM – 4:00 PM
Individual Supports Program Hours: Sunday-Saturday, 9:00 AM – 6:00 PM

Community-based position with regular travel throughout the service area

What You'll Do

Provide direct support to individuals in home, community, and program-based settings
Deliver individual supports case management, including coordination of services and community resources
Promote individual choice, self-expression, independence, and community inclusion
Facilitate assessment, implementation, and documentation of individual goals and treatment objectives
Provide skill-building support, supportive counseling, and crisis intervention as outlined in service plans
Coordinate access to medical providers, community services, and natural supports
Assist individuals in developing and maintaining meaningful relationships
Provide transportation to appointments, activities, and community-based programming as needed
Maintain accurate and timely documentation in compliance with KDC and funding agency requirements
Collaborate with interdisciplinary teams, medical professionals, and community partners
Participate in staff meetings, supervision, and ongoing training opportunities
Ensure all individuals are treated with dignity, respect, and confidentiality

Required Qualifications

High School Diploma or GED required
Valid driver's license and reliable vehicle required
Experience supporting individuals with developmental disabilities in community-based settings
Strong organizational, interpersonal, and communication skills
Proficiency with Microsoft Office and electronic documentation systems
Ability to adapt to changing schedules, environments, and individual needs
Commitment to person-centered supports and community inclusion

Preferred Qualifications

ASL fluency strongly preferred
Experience providing case management or coordinating services for individuals with disabilities
Knowledge of community resources, benefits, and support systems
Experience working collaboratively with families, providers, and interdisciplinary teams
Crisis intervention and de-escalation experience

Working Conditions & Physical Requirements

Frequent travel throughout the community is required, including transporting individuals as needed
Ability to work in a variety of environments, including homes, community locations, and program settings
Prolonged periods of driving, standing, walking, sitting, and computer use
Regular bending, kneeling, reaching, and assisting individuals with daily activities
Ability to lift, push, pull, or carry up to 40 pounds as needed
May be required to provide physical assistance to individuals with mobility needs or during crisis situations
Must be comfortable working independently in community settings and adapting to changing daily priorities
