Full Job Description

SUMMARY STATEMENT:

The Case Manager I, under the supervision of the GRYD Coordinator shall conduct outreach and referral network for GRYD services; plan, develop, and implement Gang Prevention and Youth Development activities; provide individual, group and family sessions, behavior modification, and support participants at school meetings, LAPD meetings, and probation hearings; provide supportive services to progress on the risks factors reduction as outlined in the GRYD Model of Practice Handbook; Input data entry for all services rendered to GRYD participants on the ETO system. This position is required to work on specific holidays and weekends based on assignment and contract.

B. DUTIES AND RESPONSIBILITIES:

1. CONDUCT OUTREACH, ADVOCACY AND A REFERRAL NETWORK FOR PROJECT SERVICES:

Collaborate with other CBO’s, schools and other stakeholders to provide awareness of GRYD services through Community Education Campaigns.
Provide referrals to youth and their families to services not encompassed within the GRYD project that meet the family’s goals and objectives.
Communicate with sensitivity to clients from diverse language and cultural backgrounds.
Evaluate calls and walk-in’s from potential clients and determine appropriateness for GRYD.
Prepare all documentation on compliance with Federal, State, City and Agency guidelines.
Conduct all screening, intake, contract management and contract eligibility forms for prospective project participants.
Create individual service plans for all GRYD participants.
Provide support and advocacy services for program participants including the attending of court hearings, school meetings, court, probation and DCFS.
Participate and attend team meetings, GRYD partnership meetings, community meetings, promote and implement Summer Night Lights (SNL) activities, workshops and events in the summer months.
Conduct other GRYD program duties as assigned.

2. PLAN, DEVELOP, AND IMPLEMENT PREVENTION & YOUTH DEVELOPMENT ACTIVITIES:

Plan and provide field trips, social and recreational activities, and educational outings for program participants.
Develop and maintain strategies for client referrals.
Conducts direct services to youth and their families who are eligible youth between the ages of 10-15 years old by implementing the GRYD Prevention Model of Service throughout the seven phases.
Plan community functions i.e. health fairs, resource fairs, community events, etc.
Provide transportation on as needed basis to and from events and meetings to program participants.
Transport equipment and supplies in personal vehicles to various locations.
Facilitate family, small and large group sessions.
Create, develop and implement AIYDA with all participants.
Implement Individual Service Plan, individual, group and family sessions to monitor and advise on individual service plans and the GRYD Model approach.
Identifying assets and problems using the Youth and Family Service Plan, referring participants and their families to services outside the GRYD Scope of Work (i.e. mental health, housing, etc.).

3. CASE MANAGEMENT:

Administer YSET and YSET-R assessments, facilitate developmental activities and workshops and attend mandated City Trainings.
Maintain participant data and records, activities, youth developments and support services on the ETO and hard case files.
Engage and promote GRYD participants through 10 youth development group activities per cycle (cycle = 6 months).
Provide comprehensive case management services to Model Participants by conducting individual and family meetings, strategy team meetings, building and continuously working on strength-based genograms.
Attend unit meetings, supervision, team meetings, SNL meetings and other meetings as assigned.
Administer the YSET assessment tool, enroll and engage participants.
Manage a caseload of 20 secondary and 5 primary or a total of 25 secondary, maintain case files and data on the ETO on a weekly basis.
Establish and maintain positive client relationships, build trust and respect by maintaining weekly sessions with clients and their families.

Qualifications and Education Requirements:

Four years education Bachelor's degree in related field or CAS certified with 2 years clinical experience.
Minimum of 4 years of Case Management experience and able to manage a caseload of 25 cases at any given time (Secondary and/or Primary)
Demonstrated knowledge and skill in computer operations.

Preferred Skills

Bilingual Spanish Preferred
Bilingual or bicultural capabilities with Los Angeles County threshold languages: Arabic, Armenian, Cantonese, Farsi, Urdu, Khmer (Cambodian), Korean, Mandarin, Russian, Spanish, Tagalog, Vietnamese, and other Chinese languages.
First Aid and CPR certification from a recognized agency.

General Requirements and Knowledge, Skills, and Abilities (KSA’s):

Valid Class C Driver’s license, Calif. Minimum Automobile Insurance, and an appropriate motor vehicle.
Permanent resident or U.S. Citizen; proof of work eligibility upon hiring.
Must have three (3) years of recovery or drug free lifestyle as of the date of the application.
Must provide proof of full vaccination and 1st Booster against Covid-19 upon hiring.
Ability to get along well with others, be punctual, pleasant, and tactful with professional appearance and conduct.
This position is required to work on specific holidays and weekends based on assignment and contract.
Ability to establish and maintain effective working relationships with supervisors, coworkers, clients and their families, community organizations, and the general public.
Ability to be culturally competent of age groups, gender identity, sexual orientation, spiritual beliefs, socioeconomic status, and diverse cultures.

Physical Requirements:

This class requires that the incumbent stand or walk most of the time with bending, stooping, squatting, twisting reaching, working or irregular surfaces, occasional lifting of objects over 25 pounds, and frequent lifting of 10-25 pounds. Ascending and descending ladders and staircase will be more frequent. An example is this position frequently will be moving and organizing agency supplies.
